The Care Coordinator is a highly visible customer service and patient-focused role. The incumbent in this role works directly with our patient population and their families, insurance representatives and outside vendors, physicians, clinicians and other medical personnel to ensure the referral process runs smoothly. He/She operates in a dynamic and professional environment to ensure the highest level of quality healthcare is delivered to our members.
ESSENTIAL JOB DUTIES/RESPONSIBILITIES:
Coordinates and processes patient referrals to completion with precision, detail and accuracy.
Definition of completion:
Prioritizes HPP patients in Primary Care Physicians panel, stats, expedites and orders over 5 days.
Orders have been approved (when needed).
Schedules patient (Preferred Providers List of Specialist) and notifies them of appointment information, including, date, time, location, etc.
Uses Web IVR to generate authorizations (Availity, Careplus, Healthhelp NIA and any other approved web IVR for authorization processing).
Completes orders with proper documentation on where patient is scheduled and how patient was notified.
Referrals have been sent to specialist office & confirmed receipt.
Prepares and actively participates during physician/clinician daily huddles utilizing RITS Huddle Portal and huddle guide. Effectively communicates the physicians/clinicians needs or outstanding items regarding to patients.
Enters all Inpatient and Outpatient elective procedures in HITS tool.
Ensures patient's external missed appointment are rescheduled and communicated to the physician/clinician.
Participates in Super Huddle and provides updates on high priority patients referrals.
Addresses referral based phone calls for Primary Care Physicians panel.
Completes and addresses phone messages within 24 hours of call.
Checks out patients based on their assigned physician/clinician. (Note: If assigned Care Coordinator is unavailable at the time of check out, a colleague shall assist patient. This process does not apply to Care Specialist)
Retrieves consultation notes from the consult tracking tool.
Follows up on all Home Health and DME orders to ensure patient receives services ordered.
Provide extraordinary customer service to all internal and external customers (including patients and other
ChenMed Medical team members) at all times. Utilization of patient messaging tools.
Performs other related duties as assigned.
KNOWLEDGE, SKILLS AND ABILITIES:
Knowledge of medical terminology, CPT, HCPCS and ICD coding desired
Detail-oriented with the ability to multi-task. Must be open to cross-functionally training in referrals and back office duties
Able to exercise proper phone etiquette with the ability to navigate proficiently through computer software systems
Team-oriented with the ability to work extremely well with patients, colleagues, physicians and other personnel in a professional and courteous manner

Ability and willingness to travel locally within the market up to 10% of the time
Spoken and written fluency in English; Bilingual a plus

The PCC will support administrative and business functions of the clinic by supporting the Physicians, Physician Assistants, and other personnel to ensure that all needs are being met.
Essential Job Duties and Responsibilities:
The PCC has the responsibility to carry out duties as directed by My Houston Surgeons.
Communicates on a regular basis with the designated My Houston Surgeons Supervisor(s).
Alerts Supervisor(s) and seeks assistance when an activity or process change within the clinic changes or deviates from the current process.
Follows and always maintains patient confidentiality appropriate with HIPAA compliance standards.
Schedules appointments for patients from varying avenues to include but not limited to building rapport with patient, discussing fees, and educating patient on procedures.
Answers inquiries through physician specific website email inquiries and other avenues. Documents interactions through Nextech or designated spreadsheets.
Maintains applicable consult spreadsheets, updating daily, and/or tracks scheduling tasks to ensure appropriate scheduled follow ups are conducted as outlined in separate - PCC document.
Provides brochures/materials to patient with surgeon specific related items, Breast Reconstruction brochures and any additional information as needed.
Tracks all steps of the surgery scheduling process (Consultations, Pre- and Post-Operative appointments, Surgery, Follow-Ups, etc.)
Reviews lab/clearance protocols with patients and gives order as needed. Coordinates with clinical team members as needed.
Ensures lab/clearance results are received prior to pre-operative appointment as needed. Coordinates with clinical team members as needed.
Communicates with physicians in obtaining the appropriate CPT and ICD-10 codes for all procedures.
Involves billing department to include authorization and pre-determination clearance for surgeries, any patient related financial questions. Responsible for communicating and tracking the progress of these items with the billing department.
Maintains surgeons' schedules by adding procedures/surgeries to all pertinent calendars.
Contacts surgical facilities to post and send orders, sends history and physical, follows facilities scheduling protocol, obtains quotes for facility and anesthesia fees, and confirms surgery the day before. Updates surgery orders as needed.
Coordinates surgery dates with other surgeons PCC's and books accordingly.
Coordinates surgical assists as needed per surgeon request.
Arranges special equipment for procedures to include but not limited to implants, machinery, etc. Ensures that appropriate personnel are notified as needed.
Monitors surgeon's schedules daily to assess if appointments need to be moved, any errors in scheduling, or appointments that may need additional information. Communicates with front desk and surgeons daily.
Ensures pick sheets for procedures are sent when surgeries are being done at new surgery facilities.
Attends training sessions as needed to increase knowledge base and skill level for the position.
Other duties as assigned by My Houston Surgeons supervisors and physicians.

Dow has an exciting opportunity available for a Senior Scheduling Specialist located in Houston, TX.
About you and this role:
The Senior Scheduling Specialist acts as a technical resource for Scheduling within the Global Project Methodology (GPM) work process. Supports establishment of the overall schedule baseline and reporting to monitor and control the project within established tolerances of the baseline supporting efficient project execution. Coordinates data inputs from the various disciplines and functions to ensure accurate forecasts and reporting of schedule. Engages the project steering team to understand causes of the deviation and the need to establish actions to mitigate, where deviations from established tolerances occur. Requires in-depth conceptual and practical knowledge in own job discipline and basic knowledge of related job disciplines.
Responsibilities / Duties:
Develops conceptual schedules to evaluate various alternates or anticipated schedule challenges.
Develops and maintains comprehensive integrated master project schedules including Front-end Loading, Detail Design, Procurement, Construction and Start-up/Commissioning Project Stages.
Maintains schedule during the project life cycle monitoring progress and incorporating modifications where changes in the original scope occur or delays are encountered.
Promotes discipline knowledge exchange and quality application through technical coaching.
Contributes to improvement of Schedule Management Work Process and leveraging of learning experiences.
Provides recommendations on cycle time reduction opportunities and techniques, as applied to scheduling.
Develops schedule management plans for project plans for management and control.
Utilizes project cost estimates to resource load project schedules and gain alignment with estimated scope of work and the project schedule.
Performs staffing density checks or staffing limitation checks to validate schedule durations and ensures resource-leveling supports the project strategy.
Leads project teams in forecasting schedule challenges and offers analyses and solutions to complete the project on schedule.
Performs critical path analysis to determine problem areas in regards to project schedule and resource overloads and offers alternative courses of action to the project team.
Evaluates project deviations to assess schedule impact for proper decision making and recovery plans.
Works with cost management to gain alignment when forecasting schedule completion dates.
Applies established schedule management practices to ascertain critical schedule issues and recommend corrective actions.
Defines comprehensive Schedule Management solutions for capital and expense projects.
Qualifications:
A minimum of a bachelor's degree or relevant military experience at or above a U.S. E5 or Canadian Petty Officer 2nd Class or Sergeant ranking OR in lieu of a degree 7 years of experience in project controls scheduling.
A minimum of 3 years experience with Primavera P6.
Must be willing and able to meet all applicable requirements related to business travel.
A minimum requirement for this U.S. based position is the ability to work legally in the United States. No visa sponsorship/support is available for this position, including for any type of U.S. permanent residency (green card) process.
