Description of Services
The Behavioral Health Worker will be responsible for implementing mental health treatment and crisis intervention for children and families through the Clinical Home model. This position also includes providing psycho-educational groups for youth and/or families, intensive case management service to children and families, and supporting clients with one-on-one interventions in the school, home, and community to support the overall mental health goals of the childs treatment plan. Workers will attend weekly supervision and consultation deemed appropriate as well as frequent participation in trainings.
Specific Job Duties and Responsibilities
Clinical Responsibilities:
1. Provide direct behavioral interventions and teach client-specific skills. This may include individual/group interventions, psycho-education groups, and assisting in crisis intervention to identified children and families within the school system as well as in the home and community.
2. Maintain a collaborative relationship with the parent/guardian of each client.
3. Collect and update data related to clients record.
4. Implement individualized crisis plans for each client and family. Work with treatment team to help neutralize crises.
5. Participate in on-call crisis intervention triage as needed during evenings, nights, and weekends.
Assist in debriefing of crisis situations to develop more effective plans for families served.
6. Attend and actively participate in weekly group supervision.
7. Document and complete daily case records required and in compliance with all Federal and State regulations.
8. Attend all trainings and workshops as required for professional development.
9. Provide Structural Family Therapy interventions for children and families as directed.
Case Management Responsibilities:
1. Provide intensive case management to children and families that include addressing needs that negatively impact the familys level of functioning.
2. Participate in Interagency meetings as deemed appropriate.
3. Participate in Individualized Educational Plans as deemed appropriate.
4. Complete required reports to Child line and other agencies involved, as necessary.
5. Work in collaboration with Program Director, Clinical Consultant, Administrative Staff, Quality
Assurance Department, and the billing department to assist in the process of monitoring quality assurance and other protocols necessary to effectively manage programmatic aspects.
6. Maintain high quality records sufficient to meet State and Federal clinical documentation requirements that include all funding sources.

SUMMARY

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following based on site requirements. Other duties may be assigned.
• Follow Standard Operating Procedures (SOP), IRB Protocols, and regulatory requirements.
• Complete all process and procedure training as required.
• Create a welcoming environment for donors to increase donor retention and participate in donor recruitment, as needed.
• Consent donors and perform pre-donation donor screening qualification processes according to SOPs.
• Maintain confidentiality with respect to personnel, donors, and site records utilized during all duties.
• Preparing donors for donation procedures in a timely manner.
• Review donor record files for accuracy and completeness and ensure documentation is performed according to SOPs.

• Perform daily, weekly, monthly, annual, and periodic calibration and qualification activities.
• Assist in performing donor center facility and equipment cleaning and maintenance daily, weekly, monthly, and/or annual.
• Process, label and review units and samples in preparation for shipping according to specifications, as necessary.
• Perform product and sample shipment activities, as necessary.
• Monitor collection supply levels and enter purchase requests, as necessary.

This position may require up to 5 % travel over the course of a year.

Job Description
New Perspective is now offering a sign-on bonus of $1,000 for full-time Caregiver Med Passers at our Winds community in Duncansville, PA.
(Bonus Eligibility - New Team Members Only. Paid within 1st year of employment.)
The Caregiver Med Passer, is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. This includes administration of medications, documentation of medication administration and resident care while demonstrating New Perspective Senior Living's Mission and Values in accordance with federal, state and local standards and regulations. Within the first 30 days upon your hire date, you will be required to learn and properly perform the Caregiver responsibilities and successfully complete the Med Passer training requirements. Upon completion of these requirements, you will become our next qualified Med Passer on the team.
**REQUIRED: previous Med Passer experience to qualify for the 30-day Med Passer offer.

Responsibilities:
Reads and documents in all communication logs and informs the nurse when needed
Effectively completes incident reports per company policy
Accurately dispenses, passes medications & documents in the MAR, absent of errors
Accurately assists residents with self-administration of medications
Accurately counts narcotics in accordance with company policy
Honors personhood by reading and knowing the Residents' Life Stories
Actively involved in the life enrichment program
Is compliant with company training requirements
Demonstrates a high level of safety and privacy of resident information in all work practices
Champions “follow-through” on all delegated tasks from the Director of Wellness
Assists residents in accordance with their individual service (care) plans to include: personal care and activities of daily living (eating, drinking, walking, and transferring) and use of adaptive equipment (hearing aids, glasses, and dentures)
Complies with company Mission, Values, and Principles of Service, policies and procedures, and applicable laws
Communicates and interacts in a professional, respectful, and hospitable manner with residents, families, and team members
Qualifications:
High school diploma or equivalency required
Must be at least 18 years of age
Previous Med Passer experience required
Ability to read, write, speak & understand the English language
Ability to handle multiple priorities and tasks
Ability to make decisions and act in the resident's best interest
Ability to react and remain calm in difficult situations
Preferred Qualifications:
Certified Nursing Aide (CNA)
Previous experience working with seniors preferred and desire to serve and care for seniors
Competent in organizational and time management skills

Description of responsibilities:
Review resident service plans
Assist/observe medications and treatments for each resident using the medication observation record and the Six Rights of Medication Pass
Document medication administration
Maintain confidentiality of all resident information
Report resident change of condition to supervisor
Follow refill process for medications
Count the med cart at change of shift with oncoming medication technician/ nurse shift supervisor
Take and record resident vitals
Assist residents with activities of daily living
Perform other duties as assigned
Required experience:
High School Diploma/GED
Medication Technician Certification per state regulations
CPR/ First Aid Certification
Nursing Assistant certification a plus
Must be 18 years of age
