Job Summary
The PatientCare Tech, under the direction of a Registered Nurse (RN) or Licensed Practicing Nurse (LPN) and, according to established procedures, performs patientcare tasks and patientcare procedures necessary to provide for the care, comfort and safety of patients. Demonstrates the knowledge and skills necessary to provide care appropriate to the age(s) of the patients served on the assigned unit.

Responsibilities and Essential Functions
Patient & Visitor Interactions /Customer Service/Hourly Rounding: In this role, the PCT will interact with patients and visitors, answering patient call lights, answering phones, assisting visitors within the facility. Performs purposeful hourly rounding on all assigned patients.
Patient Safe Handling/Ambulation/Repositioning: In this role, the PCT will learn patient safe handling skills to allow the PCT to assist in the ambulation of patents and to be able to assist the nursing staff in the transferring, lifting and /or repositioning of patient utilizing Safe Patient Handling equipment.
Basic Nursing Care- ADLs: In this role, the PCT will provide basic care by assisting patients with personal care needs including bathing, grooming, oral care, eating, feeding, use of bedpan or urinal, positioning, answering and responding to patient call lights, bed making, etc. under the supervision of a RN/LPN. Follows policies and procedures related to patient and staff safety.
Admission/Discharge/Transfers: In this role, the PCT will assist with room set up, welcoming of new patients/ room orientation, discharging and/or transfer of patients. Provides patient/family orientation to room and hospital according to hospital practice. Transports/escorts patient to area of destination ensuring safety; regularly updates patients.
Procedures and Treatments: In this role, the PCT will accurately complete and document patientcare activities including: ADL, vital signs, weights, (actual weight on admission or as ordered), intake & output, and calorie counts according to policy and procedure. Effectively explain and perform procedures (e.g., urinary catheter removal, ECGs, wound care and dressing changes under the direction of the nurse). Notifies appropriate personnel of response and questions. Collects non-sterile specimens such as routine urinalysis and stool specimens. Accurately performs point of carepatient blood sugar testing under the direction of the nurse. Promotes and maintains skin integrity by reporting patient problems as they occur, to the appropriate team member.
Care Companion/Sitter/Observer: In this role, the PCT may be asked by the RN or LPN to remain with a patient as a means to keep the patient safe.
Department Specific Tasks: In this role, the PCT will support the nursing team by indirect patientcare activities such as restocking equipment/linens, cleaning equipment etc. Performs unit support duties including answering and relaying telephone calls, filing and maintaining records, retrieving and delivering supplies, specimens, instruments, test results and so forth. These indirect patientcare activities are identified in each nursing department.

Key Responsibilities
Patient Supervision & Safety
Provide continuous patient supervision and conduct scheduled safety checks to ensure patient and facility safety.
Monitor patient behavior and environmental conditions, document observations, and promptly report concerns, incidents, or changes to clinical or medical staff.
Respond to crisis situations using approved de-escalation techniques and follow emergency protocols as trained.

Monitoring
Measure and document vital signs as assigned and perform approved CLIA-waived testing, including urine drug screening and pregnancy testing, in accordance with training and policy.

Observe and report physical, emotional, or behavioral changes that may impact patient safety or treatment progress.
Patient Interaction & Program Support

Support patient engagement in treatment programming and daily routines within the milieu.

Collaborate effectively with clinical, nursing, medical, and operations staff to maintain a safe and supportive treatment environment.
Assist with patient transportation when required by assignment and ensure compliance with safety and driving requirements.

Qualifications
High school diploma/equivalent or higher.
Minimum age of 21 and ability to meet company motor vehicle insurance requirements; valid driver's license and acceptable driving record required when driving is assigned.
Experience in behavioral health, substance use disorder treatment, and/or mental health settings strongly preferred.
Experience working in detoxification and/or residential levels of care preferred.
Behavioral health certification or related education preferred.
Ability to maintain professionalism, empathy, and composure in high-stress or crisis situations.
Strong observation, communication, and documentation skills.
Ability to work effectively as part of a multidisciplinary treatment team

Technician - Mount Sinai Hospital Hemodialysis Unit
Mount Sinai Hospital hemodialysis unit is one of the few remaining hospital-based dialysis programs in the country. The hemodialysis unit has always been maintained by the hospital since the unit first opened in the 1960s.
The dialysis program currently includes dialysis services at Mount Sinai Hospital, Schwab rehabilitation hospital, and Holy Cross hospital. At Sinai, the services provided are outpatient and inpatient hemodialysis; at Schwab, inpatient hemodialysis; and at Holy Cross, inpatient hemodialysis.
The outpatient dialysis unit has 11 stations, serving four shifts of patients Monday through Saturday. There are two staff shifts. The inpatient unit has 4 stations and also provides bedside dialysis. Schwab has a 2-station unit and also provides bedside dialysis. Holy Cross has a 6-station unit and provides bedside dialysis as well. All acute services also are operational Monday through Saturday. The unit is closed on Sunday. Acute staff has on call responsibilities. Also, Mount Sinai hospital and Schwab are currently developing a stronger peritoneal dialysis program.
The staffing mix is approximately 60% RN and 40% certified hemodialysis technicians. The chronic staff work five, 8 hour shifts and the acute staff work four, 10 hour shifts.
Mount Sinai hospital has four nephrologists, all part of the Sinai health system. They are on site, responsive, and supportive of the dialysis staff.
We enjoy our independence functioning within a hospital system, as opposed to an outside management company. Our dialysis patients can more easily coordinate their other appointments and services along with their dialysis treatments here at Sinai. The unit also has practically no staff turnover. There are numerous learning opportunities from the different dialysis modalities as well as learning about all the equipment that is involved in preparing a safe dialysis treatment.

Replaces pulse oximeter probe on patient's finger; observes pulse oximeter readings while providing care and notifies nurse or RCP of low readings. Performs simple procedures such as non-sterile dressing changes under the direction of a registered nurse.
Performs common bedside tests including blood glucose monitoring, and checking stools for occult blood. Reports findings immediately to the patient's nurse, and documents in Meditech.
Observes and reports patient signs and symptoms and responses to treatments. Reports changes in the patient's condition to the registered nurse and respiratory therapist.
Assists with admissions and discharges. Orients patient and family to room, unit and environment. Handles patient belongings carefully and records them upon admission. Collects and packages patient belongings at discharge.
Observes or reports the emotional and spiritual needs of patients and assists in providing supportive care.
Answers call lights and responds to needs for all patients, not just those assigned. Assists other staff members in providing care to any patient on the unit. Offers assistance to other staff without being asked.
Performs related services and duties such as cleaning equipment, obtaining and replacing patient room supplies, and similar related activities. Ensures that all supplies used are charged according to policy.
Insures the enhancement of personal knowledge and skills by participating in continuing education programs and reading pertinent literature, etc.
