The salary range for this job opening at PeaceHealth is $46.60 - $88.26. The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc.

**Job Summary**
The RN Critical Access is a team member who provides professional nursing direct patient care in compliance with professional practice and unit standards. Promotes a collaborative, interdisciplinary approach towards patients, families and all members of the care delivery team.
**Essential Functions**
+ Assess, plan, implement and evaluate patient care needs.
+ May coordinate admissions, discharges and transfers; communicates and prioritizes, and provides for patient privacy, exercises discretion and assures confidentiality at all times.
+ Works collaboratively and professionally with the physicians and leadership in the delivery of patient care and problem solving within the department.
+ Assures continuous patient care including, but not limited to, overseeing the stocking of supplies, medications, and patient care items; staff breaks and lunches; timeliness of patient flow throughout the department and ancillary departments.
+ May perform limited x-ray examinations, point of care testing and other procedures as determined and will apply, appropriate infection control and radiation safety.
+ Participates in quality and process improvement initiatives through gathering of data, staff education and monitoring utilizing the tools recommend by the organization.
+ Ensure professional effectiveness through education and professional development.
+ Will be required to perform patient transport functions.
+ Performs other duties as assigned.
**Qualifications**
**Education**
+ Degree from an Accredited School of Nursing
+ Preferred: Bachelor of Science in Nursing
**Experience**
+ Required: Minimum one year of recent experience in an EmergencyDepartment or Urgent Care Setting.
+ Required: Demonstrated competency to perform procedures
+ Successfully completes the non-violent crisis intervention training in orientation and renews training as outlined in policy.
**Credentials**
+ Required: RegisteredNurse in state of practice

3) Schedules all transtelephonic checks via EMR system and notifies patients.
4) Reports abnormal findings, patient complaints, or concerns, to physician or supervisor.
5) Assures the accuracy and completeness of messages. Triages and prioritizes messages, communicating information to providers/patients in a timely manner. Demonstrates triage skills pertinent to determining normal and abnormal conditions. Reviews information regarding the patient's device, health history, medications and all information pertaining to the patient's heart failure status.

Legacy Salmon Creek Medical Center is Southwest Washington's most modern hospital, offering the latest technology in a setting designed for comfort and care for the whole family. Salmon Creek Medical Center is certified for Level III neonatal care. Our NICU is able to care for 25 critically ill newborns. Our unit features 15 private rooms, three rooms for twins, and space for four more babies in our special care nursery. Family space, including a sleeping area is built into each family room.
Responsibilities
Responsible for patient care within an assigned department/unit and participates as a member of the health care team in cooperation with and under the direction of the clinical manager or designee as defined by the relevant Standards and Scope of Practice for the RegisteredNurse. The RN Staff Nurse assesses, analyzes, plans, implements and evaluates patient care following established patient care standards for Legacy Health System including those for patient assessment and transfer.
Qualifications
Education:
As required by licensure. BSN or degree in progress strongly preferred. BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO.
Experience:
One-year registerednursing experience or completion of either the Legacy Health RN Residency program or a nursing program that is equivalent in content, curriculum and duration. Additional clinical experience requirements may vary dependent upon the specific department in which the position is utilized.
Recent NICU experience strongly preferred.
Knowledge/Skills:

Major Responsibilities:
Uses the nursing process to provide ongoing care (assess, plan, intervention, and evaluation). Recognizes and responds to changes in a patient's status and need for care. Determines priorities of patient care based on essential patient needs.
Provides triage and consultation to patient, family, and/or significant others according to protocol and organization guidelines. Communicates with physicians and other members of the health care team. Documents and communicates accurate and timely information regarding patient conditions.
Prepares and rooms the patient for exam by obtaining vital signs and documenting/updating pertinent health information (i.e. chief complaint, allergies, medications).
Performs laboratory procedures (i.e. strep test, wound culture, specimen collection, etc.) using principles of aseptic technique and standard precautions / infection control guidelines. Administers medications, under physician/provider orders, including: immunizations, antibiotics, narcotic pain medications, vitamins, topical agents, insulin, blood thinners, and allergy antigens. Initiates, maintains and discontinues IV therapy; performs mixing of medications according to protocols.
Assesses and provides basic to complex patient education and counseling on the performance of tasks, skills, treatment and self-care integrating health promotion and preventive aspects of care. Follows through with necessary procedure or test requests, prescription refills, new appointment times and referrals to other facilities or services. Refers questions to physician/provider and relays information back to patient as directed.
Performs advanced clinical tasks or skills such as removal of sutures and staples, catheterization, audiograms, laboratory procedures (throat/nose culture, drug screen, phlebotomy - butterfly, venous access, heel stick), spirometry, Holter monitor application, casting, complex wound care, fetal heart tone monitoring, Hickman/port-a-cath/PIC line, central line care, conscious sedation, anti-coagulation clinic operation, etc., with demonstrated competency and under clinical supervision.
Assists in the development and implementation of staff orientation, competency and educational activities. Acts as an expert clinical and department resource. May provide work direction to other staff and/or assist with patient care flow and the provision of services.
Organizes and prepares exam rooms, lab and work areas including: cleaning, stocking, ordering supplies, and maintaining equipment.

Provides work direction to the clinical and support staff with responsibility for the coordination of resources to efficiently support procedures, day-to-day operations and the smooth flow of patients through the department.
Provides support for direct patient care by performing the nursing process to deliver safe, therapeutic, and quality care through assessment of patients, establishment and implementation of the nursing care plan, and evaluation of clinical outcomes when necessary. Promotes an environment conducive to the delivery of safe, efficient and quality patient care, which may include performing patient rounding on a daily basis.
Coordinates the needs of the patient care team by initiating service complaint follow-up when necessary and promoting communication among patients, families, hospital departments and medical staff to optimize the interdependence of team members.
Acts as a resource to employees, which includes training/orienting, providing day-to-day work direction, and giving input on performance. Assigns, monitors, and reviews progress and accuracy of work, directs efforts and provides guidance on more complex issues in collaboration with the unit leadership.
Assists with human resource responsibilities, which may include: interviewing and selection of new employees, resolution of employee concerns, and employee moral or other duties as assigned in collaboration with unit leadership.
Monitors employee assignments; maintains staff scheduling, evaluates staffing levels and makes adjustments as necessary to maintain productivity level established for assigned area(s).
Monitors the quality of care provided. Assists in ensuring policies and procedures are followed in compliance with the medical center, the Nursingdepartment, and regulatory and peer review agencies.
Provides input to budget and ensures adequate supplies and equipment and inventory are available for both the delivery of safe patient care and a safe work environment.
Works with management in developing new and existing programs and services.
Works with vendors for requested products and coordinates in-services for equipment. Participates in product trials, and submits recommendations to management in collaboration with the unit Educator or CNS.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:
RegisteredNurse license issued by the state in which the team member practices, and
Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA) needs to be obtained within 6 months unless department leader has determined it is not required.
Education Required:
Bachelor's Degree in Nursing.
Experience Required:
Typically requires 5 years of experience in clinical nursing within an acute care setting.

Physical Requirements and Working Conditions:
Must be able to sit, stand, walk, lift, squat, bend, reach above shoulders, and twist frequently throughout the workday.

Major Responsibilities:
Under the observation of the RegisteredNurse, completes activities of daily living, vital sign completion and blood glucose monitoring.
Collaborates with the RegisteredNurse in establishing a plan of care based upon an analysis of health, education and developmental needs and patient/family preferences.
Under the observation of the RegisteredNurse, evaluates patient response to interventions and effectively communicates observations and patient needs to the RegisteredNurse.
Develops delegation skills appropriate to Health Unit Coordinators, Nursing Assistants and technical roles on unit.
Administers medication as delegated by the RegisteredNurse. The level of clinical judgment and proficiency of skill must be evaluated prior to medication administration.
Manages an increased number of patients and incorporates principles of time management into shift routine.
Demonstrates an ability to collect and synthesize relevant patient data accurately identifying patient status and needs.
In emergency situations and under appropriate supervision of a RegisteredNurse, participates in activities they have demonstrated knowledge and skill in.
Performs various patient positioning and transporting duties, which require lifting and pushing/pulling, while utilizing proper technique.
Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs, and to provide the care needed as described in the department's policies and procedures. Age-specific information is developed further in the departmental job standards.
Licensure, Registration, and/or Certification Required:

Education Required:
Currently enrolled in an accredited or approved program in Nursing.
Experience Required:
No experience required.
Knowledge, Skills & Abilities Required:
Current enrollment (in final year) or graduate of an accredited nursing program.
Successful completion of a Medical Surgical Course that includes pharmacological concepts.
Good interpersonal skills and the ability to adapt to interruptions and changing priorities/workload necessary.
Physical Requirements and Working Conditions:
Must be able to sit, stand, walk, lift, squat, bend, twist, and reach above shoulders during the work shift.
Must be able to:
lift up to 50 lbs. from floor to waist.
lift up to 20 lbs. over the head.
carry up to 40 lbs. a reasonable distance.
Must be able to:
- push/pull with 30 lbs. of force.
- perform a sliding transfer of 150 lbs. with a second person present.
Will be required to use foot pedals on carts or machines.
Must have functional vision and hearing.
Will be exposed to mechanical, electrical, chemical, and radiation hazards as well as blood and body fluids on a continuous basis; therefore, protective clothing must be worn as necessary.
