Peak Vista CHC company history timeline

1971

Peak Vista Community Health Centers has proudly served Colorado communities since 1971.

1971: Began as an all-volunteer organization, open just two nights a week for walk-in patients

1979

The Accreditation Association for Ambulatory Health Care (AAAHC) is a private, non-profit organization formed in 1979.

1988

1988: Opened Homeless Health Center, as the Sierra Madre Clinic, with Federal funding

1993

1993: Changed name to Community Health Centers and expanded services to Homeless

1995

1995: Became Colorado's first community health center to receive accreditation through the Joint Commission on Accreditation of Health Care Organizations (JCAHO)

2003

2003: Opened newly remodeled Women’s Health Clinic

2005

2005: Held inaugural Breakfast of Champions® fundraiser (featuring Mary Lou Reton and honoring Bishop Richard Hanifen as the 2005 Champion of Community Health)

2005: Became accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC)

2007

2007: Established a designated Outreach Department to support Peak Vista’s focus on working effectively with community agencies and helping individual patients navigate health care systems and processes

2007: Included as a partner in the new Myron Stratton Consortium.

2012

2012: The Falcon Peak Health Center within Falcon School District 49 opened part time and expanded to full-time

2015

2015: Celebrated 11th Annual Breakfast of Champions® event featuring Olympian Gary Hall

2015: Advanced Practice Registered Nurse Fellowship (APRN) welcomes inaugural class of five Fellows

2015: Health Center at Fountain began hosting free monthly Mobile Food Pantry in partnership with Care and Share Food Bank for Southern Colorado

2016

2016: Mobile Health Services Van (formerly known as the Homeless Mobile Van) increased its reach by adding 2 new locations; Catholic Charities and Urban Peak in order to assure access to medical care for the at-risk and/or homeless population

2016: Celebrated 12th Annual Breakfast of Champions® event featuring Olympian Mia Hamm

2016: Graduated APRN inaugural class of fellows; four of the five Fellows stayed with Peak Vista in our family health centers

2017

2017: Celebrated 13th Annual Breakfast of Champions® event featuring Olympian Amy Van Dyken

2017: Adopted Pediatric Associates, P.C. and renamed to Pediatric Health Center at Academy

2017: Well -Child Waiting areas named to honor Julie Penrose, Spencer Penrose and El Pomar Foundation

2018

2018: Our first National Institute for Medical Assistant Advancement (NIMAA) graduation ceremony

2018: Pain and Wellness Center introduced at 340 Printers Parkway

2018: Rondald McDonald Care Mobile partnership ended due to continuing decline in patient encounters

2018: Homeless Health Center name changed to Health Center at Rio Grande

2019

2019: Moved Health Center at Union to the 15k sq ft newly renovated Health Center at Jet Wing

2020

2020: Opened our community-based Family Health Center at Mitchell High School; a partnership with Colorado Springs Disctirct 11
