Saratoga Hospital Medical Group - Pain Management, seeks a full time NP/PA, to join our team in a collegial environment with a highly experienced team: an Interventional Pain Management physician, a PM&R physician and PhysicianAssistant with exceptional support staff.
We provide comprehensive Interventional Pain Management services, prescribing medication management without assuming primary opioid prescribing responsibility and education in an ambulatory setting in Saratoga Springs, just off Exit 15 of Route 87. The center is located upstairs from Saratoga Surgery Center.
The advanced practitioner in this role will perform initial history and follow-up examinations and provide ongoing care, order diagnostic tests and consultations, and provide education and counseling to patients and families. Excellent communication skills are required with patients, colleagues, and providers. Requirements include: NYS-licensed Nurse Practitioner or PhysicianAssistant. Experience in primary care or prior pain management experience, is preferred.

SUMMARY
The APC Collaborative Teaching Pod is a unique and innovative opportunity for learning value-based primary care. The incumbent will work with three-four advanced practice clinicians who are either new graduates or new to primary care. This team or ?pod? will work together to co-manage a panel of patients, supported by a supervising physician and a designated MD or APC preceptor for each session. Didactic sessions will occur twice a month along with opportunities to learn from colleagues in medical specialties. The teaching pod is a one-year commitment with the potential for job placement within Atrius Health or Reliant Medical Group after successful completion of the program. Placement in a permanent position will depend on hiring needs within the respective organizations at that time.
The incumbent is a certified nurse practitioner (NP) who possesses advanced training/skills in assessing, diagnosing, and treating the patient with both chronic and acute illness and disease. Works in a collaborative team practice that recognizes the patient as the central focus. Serves as a patient educator in areas of specialization.
GENERAL DUTIES AND RESPONSIBILITIES
Diagnoses and treats acute and chronic illnesses that are of an appropriate level of complexity for the knowledge and experience of the NP.
Orders and interprets laboratory work and other diagnostic studies. Develops and implements a plan of care to address the illness, which may include prescribing medications and/or other treatments. May refer patient to subspecialty physicians.
Teaches and counsels individuals and groups regarding specific health care needs. Serves as an educator in areas of specialization (e.g. family planning, weight loss, smoking cessation).

EDUCATION/LICENSES/CERTIFICATIONS
Graduate of a State-approved school of Nursing and Nurse Practitioner program. Master of Science degree and Nurse Practitioner certification in an area of specialty required. Possess a current license to practice as a professional nurse in the Commonwealth of Massachusetts, and authorization to practice in the expanded role. Possession and maintenance of or eligibility for DEA and Mass DPH Controlled Substances Registration. American Heart Association Basic Life Support (BLS) required. Per Diem PhysicianAssistant - Hospitalist PA - Internal Medicine
The per-diem hospitalist PA will be utilized to cover our inpatient med-psych unit, inpatient rehab unit, and inpatient medicine services. The per-diem PA will have their own panel of patients on one of the non-teaching services providing general inpatient management care, including admissions/discharges, medication reconciliation, patient counseling, and other daily management and care.
Coverage currently includes all shifts. Priority coverage needs for day shift (7a-8p) and night shift (7p-8a).

Responsibilities
Responsibilities include evaluation and management of acute/chronic illnesses, ordering/interpreting/communicating diagnostic tests and results, hospital/ED follow-ups and routine well visits, transition of care management, collaboration with physicians and medical team regarding patient care, facilitating appropriate referrals to specialists or community-based agencies. Opportunities in patient education/counseling intended to maintain or restore physical/emotional health and function, and educational support for various learners consistent with our goals.
Excellent communication and collaboration skills are essential with the ability to exercise independent judgement and initiative.
The candidate must have completed NYS NP or PA certification along with licensure and have applied for DEA registration prior to hire/start date.
Schedule: 4 Day Work Week
Monday, Tuesday, Thursday, Friday (OFF Wednesday)
8:00 a.m. to 4:00 p.m.
On-Call: 1 in 6 and there will beill an additional stipend paid when on-call.

Essential Job Functions
· Ensures that each patient receives outstanding service by providing a friendly environment, which includes greeting and acknowledging every patient, maintaining outstanding standards, solid product & program knowledge and all other aspects of customer service
· Conducts patient calls to include: appointment reminders, care calls, progress/patient concerns and lab results
· Obtains and review patient?s medical and psychosocial history information relevant to patient care
· Conducts physical examinations; orders/reviews laboratory results & diagnostic procedures; including EKG
· Documents patient progress notes in the patients EMR chart
· Assumes responsibility for prescribing and/or dispensing FDA approved appetite suppressants according to state and federal guidelines
· Maintains all required documentation for patient care and medication management
· Review patient?s progress and manages patient care accordingly
· Facilitates referrals to other healthcare professionals and medical facilities as necessary and will communicate with collaborating physician and/or specialist regarding patient?s care
· Stays in contact with Medical Affairs regarding the implementation of patient care policies and other general location policies and procedures
· Follows Medical Director requirements for chart review per the Collaborative Agreement
· Participates in and assists with interviewing, hiring and evaluation of location personnel.
· Conducts consultations and Initial Assessments with patients regarding the Medi-Weightloss? program(s)
· Adheres to and stays current on all Medi-Weightloss? program system standards
Non-essential Job Functions
· Participates in location marketing efforts and provides ongoing support to promote location profitability & success

· Utilizes phone system to answer and appropriately route all incoming calls when other staff are unavailable
