Responsibilities
* Performs initial nursing evaluation, including behavioral health/psychiatric nursing needs.
Observes and evaluates the physical and mental status of the patient for complications and medication side effects.
Maintains awareness for indicators in patients' behavior that call for change in the treatment modality (crisis, suicidal ideation) as well as other medical signs and symptoms.
Instructs patient/caregiver in cognitive and technical aspects of care, medication regimes, disease processes, safety interventions, ADL care, coping skills, symptom management and other interventions.
Administers IM medications, performs lab draws and specimen collections.
Acts as a case manager in coordinating care for patients as assigned.
Completes documentation timely and accurately.
Supervises LPNs and HHAs.
Regularly communicate patient progress to the clinical manager, psychiatrist and care team.
Qualifications
Current RN license, specific to the state(s) you are assigned to work. Must have one of the following to meet CMS requirements for psychiatric nursing in the home care setting:
An RN with a master's degree in a mental health nursing or psychiatric specialty. Must have nursing experience in an acute treatment unit or other psychiatric setting.
An RN with a bachelor's degree in nursing. Must have one year of recent nursing experience in an acute treatment unit or other psychiatric setting.
An RN with a diploma or associate's degree in nursing. Must have two years of recent nursing experience in an acute treatment unit or other psychiatric setting.

ESSENTIAL DUTIES:
Perform selected nursing procedures, commensurate with experience and education within the scope of LPN/LVN practice including but not limited to: tracheostomy care, colostomy care, enemas, douches, catheterizations, dressing changes, and phlebotomies.
Provide patient and family teaching according to plan outlined by the RN.
Monitor patient's vital signs and make specific measurements and observations.
Monitor and record intake and output. Report significant changes immediately.
Observe, assess and record patient's physical and mental condition and reports any changes.
Administer medications as instructed and as ordered by the physician and instructed by the RN. Document the patient's file accordingly.
Work with patient, family and health care team members effectively to coordinate patient care.
Maintain a high degree of confidentiality at all times due to access to sensitive information.
Maintain regular, predictable, consistent attendance and is flexible to meet the needs of the agency.
Follow all Medicare, Medicaid, and HIPAA regulations and requirements.
Abide by all regulations, policies, procedures and standards.
Performs other duties as assigned.
POSITION REQUIREMENTS & COMPETENCIES:
Graduate from an accredited school of practical nursing
Licensed as a vocational/practical nurse in the state of practice
At least two (2) year clinical practical nursing experience
Ability to make home visits and provide nursing care in the home environment
Ability to complete necessary documentation appropriately, legibly and timely
Interpersonal, verbal, and written communication skills necessary to complete job responsibilities
Must have reliable transportation, current driver's license and appropriate automobile insurance; or ability to take public transportation
