Provides service within the scope of practice as defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence based techniques, and in coordination with other members of the health care team. Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care. Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition. Instructs and informs participating members of the health care team, the patient and the family/caregivers, regarding the plan of care, functional limitations, and progress towards goals. Visits patients within the entire service area of their respective agency and other nearby LHC agencies as needed. Completes daily progress notes that reflect patients' responses to treatment, actual performance, and an assessment as to status relevant to the treatment plan in addition to eventual discharge needs and treatment summaries if needed. Completes discharge notes that include a summary of the course of treatment, progress toward goals, discharge needs, recommendations for follow-up and patient and/or family education. Documents clearly, concisely and legibly within established timeframes (e.g. allows other therapists to continue treatment, allows for interpretation by payers, supports reimbursements). Documents evidence of patient and family/caregiver education. Contacts families, physicians or other healthcare team members in anticipation of special needs of the patients or families in a timely manner to facilitate effective treatment plans. Physical Therapist Additional Requirements: Certification - Basic Life Support Upon Start Licensure - Physical Therapist VA licensure or compact privileges are acceptable Experience - Not required - at least a year in outpatient Pediatrics preferred Education - Bachelor's Degree in Physical Therapy- Masters Preferred
