Social Worker (LMSW) Position Summary
The primary function of the social worker is to coordinate the patients care collaboratively across all departments, disciplines and providers. Ensures appropriate resource utilization while providing appropriate and safe discharge planning. The ability to mentor and supervise in terms distinguishes this position from the undergraduate social worker.
Social Worker (LMSW) Position Requirements
Education
* Graduate of a master's degree program in social work from a college or university approved by the Council of Social Work.
* Licensure as a Social Worker in the state of MD required.
Experience
* Minimum of 3-5 years previous experience in social work or related capacity preferably in medical/health setting preferred.
Social Worker (LMSW) Work Schedule
* Day shift Monday-Friday, Evening and weekend work as assigned.

* At TidalHealth, full-time employees working at least 72 hours per pay period, part-time employees working at least 36 hours per pay period and part-time employees working at least 30 hours or more on weekends only are eligible for benefits.

Master's Degree from accredited school of Social Work required; Valid license/registration of permit to practice in state(s) of agency operation if required

Unless otherwise dictated by the state, at least two (2) years of professional social work experience (post-graduate) in a healthcare setting required; 2+ years preferred
Experience or education in grief counseling preferred.
Discharge planning experience preferred.
Responsibilities:
As a Hospice Clinical Social Worker, you will:
Assist the core members of the pre-hospice/hospice team in understanding significant social, spiritual, and emotional factors related to the patient's health, to establish a plan of care which fosters the personal worth, spiritual well-being, and dignity of each patient.
Participate as a member of the interdisciplinary team and in the development and review of the plan of care for all patients.
Assess the social, spiritual, and emotional needs/factors in order to estimate the patient's and involved caregiver's capacity and potential to cope with the problems of daily living and with the terminal diagnosis and illness.
Prepare the patient to cope with the changes and the chosen family to support the patient including education on advance directives/advanced care planning
Utilize all available resources, such as chosen family, hospice, and community agencies, to assist the patient and chosen family to live better within the limitations of the illness
Support the bereavement program
Provide discharge planning related to change of level-of-care or community placement/location-of-care
Provide supervision of interns and/or social work fellows (SWF), as needed. If providing supervision of SWFs, must meet state regulations as a qualified supervisor.
Required Certifications and Licensures:
Licensed to practice as a clinical social worker in the state of agency operation if required
Must be a licensed driver who can travel to all business locations

THE CARE MANAGEMENT MODEL:
Our Care Management Triad Team Model is a collaboration between the following:
RN Case Manager - manage patient care and drive patient progression and establish a discharge plan.
Social Worker - resolve psycho-social barriers and supports discharge needs.
Utilization Management - review patient status for appropriateness and anticipated payer coverage.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Master's degree in Social Work (MSW) or Master of Social Services degree (MSS) from an accredited graduate school required prior to hire date.
Licensed CSW or MSW as required by the state of Delaware prior to hire date.
Acute Care Social Work experience preferred.
SPECIAL REQUIREMENTS:
CCM preferred within 18 months of eligibility.

The Family Advocate helps the non-offending caregiver to identify and access services the family may need. The Family Advocate encourages the non-offending caregiver to participate in the investigation, prosecution, treatment and support services. The Family Advocate participates in the multidisciplinary team and provides ongoing follow-up support for the child and the non-offending caregiver. Key Responsibilities:
Greets and orients the child and family to the process.
Provides crisis intervention and support for the child and family at all stages of the investigation and prosecution.
Consults with and provides support to the Forensic Interviewers and Child Development team.
Meets and networks with referral agencies
Requirements:
Education: Bachelor's degree required (preferred in areas of Counseling, Social Work or Psychology)
