Performs or assists patient with personalcare activities as prescribed in the plan of care, including but not limited to hygiene, mobility and elimination. If unable to perform a certain task, reports to the RN or Director of Clinical Services immediately.
Provides and maintains safe environment for patient. Uses equipment safely and properly (footstools, side rails, Oxygen, etc.).
Practices Universal Infection Control Precautions in all contacts with patients.
Performs or assists patient/family with home maintenance activities as prescribed in the care plan, including but not limited to light housekeeping and meal preparation.
Monitors adequate fluid and nutrition intake, temperature, pulse, respiration, skin and basic elements of body functioning and changes in body functioning and record/report appropriately.
Practices safe transfer, ambulation, normal range of motion and positioning techniques.
Respects the autonomy and right to confidentiality of all patients and families.
Document scare provided and response of consumer/family. Submits documentation in a timely manner.
Performs within the limits of preparation and experience.
Ensures compliance with local, state and federal laws, Medicare regulations and established agency policies and procedures.
Manages a caseload and delivers care in a cost-effective manner.
Consistently meets or exceeds 365 Health Services' established service standards.
Conducts all business activities in a professional and ethical manner.
Uses supplies and equipment effectively and efficiently.
Completes a minimum of 12 hours of in services per year.
Maintains regular attendance.
Adheres to 365 Health Services' Code of Conduct.
Interacts with all staff in a positive fashion supporting 365 Health Services' mission and vision.
Conducts all business activities in a professional and ethical manner.
Complies with 365 Health Services' general orientation and with agency policies and procedures including confidentiality and HIPAA guidelines.

A DAY IN THE LIFE OF A DIRECT SUPPORT PROFESSIONAL (DSP) / CAREGIVER
As a Direct Support Professional (DSP) / Caregiver, you begin each shift ready to help adults with developmental disabilities live their best lives as independently as possible. You have made it your mission to ensure the comfort and safety of each client. In our residential group home, you assist our residents with activities of daily living such as administering medication, cooking meals, performing personal hygiene, and transportation to medical appointments.
You also assist in the implementation of individual behavioral plans. Using effective teaching and mentoring techniques, you teach them new skills and help them achieve goals. Based on each individual's preferences and behavioral plans, you encourage their interests and drive them to community outings and recreational activities. You are happy to help them find greater access to the community. You are conscientious about reporting any concerns about their health and safety and keeping documentation accurate and up-to-date.
Some aspects of this job are not easily accomplished but at the end of each shift, knowing your clients are happy makes you feel great about the work you do!
WORK SCHEDULE
We operate 24-hour residential group homes which means our Direct Support Professionals (DSPs) / Caregivers have many shift options including mornings, afternoons, evenings, overnights, and weekends.
Day shifts:
* 7 am - 3 pm
* 3 pm - 11 pm
Overnight shifts:
* 11 pm - 9 am

What will I do as a Resident CareAssistant?
Provide personalcare services such as bathing, grooming, toileting, ambulation, etc. for residents according to the resident's assessment and service plan (ASP).
Assist residents with housekeeping and laundry services.
Accompany residents on outings to medical appointments.
Assist residents with activities.
Demonstrate awareness of and sensitivity to the needs of residents and their families.
Complete documentation of resident service logs, etc.
Communicate residents' concerns and needs to the Resident Care Coordinator, Wellness Nurse, and/or Wellness Director.
What is required in this position?
A high school diploma or GED
18 years or older
Must have a valid and current CNA certification or willing to complete on-line care giving training once hired
Complete annual training as required by current regulations
Maintain First Aid and CPR certification
Excellent communications skills
