About the Position
We are looking for a Residential Care Associate I to join our team at our Columbia Rose RTF in Portland, Oregon! The Residential Care Associate I will provide caring companionship and assistance with daily tasks to clients. They will provide personalcare services and support that helps individuals develop appropriate daily living skills and independent activities of daily living to increase or maintain their level of functioning. The Residential Care Associate I will assist with meal preparation, clean-up, and food purchasing, while encouraging client participation. Under the guidance of a Registered Nurse, they will pass medications and perform delegated medical care as prescribed by the client's primary care provider. They will transport clients to and from the program as necessary to engage and provide treatment services. They will maintain compliance with established productivity standards for documentation of services.
This position falls under the AFSCME bargaining unit. Wages, benefits, and working conditions are set in accordance with the collective bargaining agreement.
Training Program
With our MHACBO-accredited training program, you will earn CEUs towards applicable mental health certifications, while attending your required, paid training, and receiving qualified clinical supervision gaining on-the-job experience.
Work Schedule: Tuesday, Wednesday and Saturday, 11:30am - 8:30pm (Part Time, Swing)

$20.60 - $21.86 per hour DOE/Credentials.

About the Program
Columbia Rose is a co-ed, 9-bed Residential Treatment Facility serving adults living with mental illness. The home features beautiful hardwood floors throughout and cathedral ceilings. All 9 residents have private bedrooms, a private deck, and a partial private bath.

What You'll Need
Caregiving and/or mental health experience (personalor professional) is preferredA dedication to the wellbeing of each resident and helping them complete their activities of daily living (ADL's) and independent activities of daily living (IADLs).
Driving is an essential function of this position. Successful applicants must provide a valid driver's license from their state of residence or obtain and submit one within 30 days of hire, maintain a safe driving record, and have the ability to pass a DHS criminal background check.
Physical Demands: This position requires the ability to frequently talk, listen, use hands and fingers, reach, stand, walk, sit, bend, stoop, climb stairs, as well as lift, carry, push, and pull up to 40 pounds. It may require the ability to occasionally use ladders, squat/kneel, and perform other physical tasks as applicable. In addition, you must successfully complete crisis de-escalation and CPR trainings, which require performing various physical, hands-on maneuvers and techniques and may require an individual to lift and transfer clients.

PHYSICAL REQUIREMENTS + WORKING CONDITIONS: Work is performed in an environment that is quite noisy and busy, and may include exposure to biological conditions including body fluids, germs, and childhood illnesses. This position is not required to provide personalcare (diaper changes), nor does it require handling of food. Physical requirements may include lifting children or heavy items (up to 50 pounds unassisted), bending, stooping, reaching, grasping, standing, and walking. Work requires vision and hearing within normal ranges.

Summary
Under the direction of a registered/licensed nurse (RN), is a member of the interdisciplinary health care team assists in clinical support functions to promote a safe, compassionate patient care environment.

Essential Functions
* Perform patient care-related duties and clinical functions as assigned, including, but not limited to, performing and documenting point of care, vital signs, EKGs, bathing and other sanitary needs of patients, transferring and assisting with ambulation of patients, feeding patients, etc. Assist RNs and physicians with other exams and treatments as required.
* Ensure that patient rooms are stocked, equipped, and cleaned appropriately, as needs indicate and as patient volumes change.
* Performs specimen collection and documents all appropriate information.
* Retrieves equipment from other areas, sets up equipment, cleans equipment between patient use, and reports malfunctioning equipment per organizational policy.
* Documents patient information, such as height, weight, intake of fluids and food, output of urine/stool/vomit/other drainage, and blood glucose results.
* Assist patients with food tray setup and eating if necessary.
* Communicate with nursing staff in an effective and timely manner around issues related to patient care and patient condition.
* Receive and direct Medical Staff, personnel, patients, and visitors.

High School Diploma or Equivalent preferred

Basic Life Support [BLS Certification] - Data Conversion - Various Issuers preferred Nursing Assistant [LNA - New Hampshire] - New Hampshire Board of Nursing preferred
Experience
Healthcare experience 1-2 years required
Knowledge, Skills and Abilities
* Ability to multitask and handle stress crisis situations effectively.
* Demonstrated knowledge of medical terminology required.
* Demonstrated typing skills and computer experience required.
* Good interpersonal, organizational, and oral and written communication skills required.
* Ability to lift and transport patients using the appropriate physical technique in a safe manner for the patient and self according to Safe Patient Handling Guidelines.

Direct Support Professional (DSP) / In Home Care Giver Responsibilities and Duties:
This role involves caring for a client on an individual basis in their home environment as well as taking them out into the community to learn skills and socialize. This role varies depending on the client but can involve teaching skills of independent living such as self-care, behavior management, budgeting, cooking, shopping and cleaning. Family Relief Resources is not a physical facility -100% of activities happen in the community or a client's home.
The day-to-day duties of the DSP role requires:
Smart Phone
- The state requires all client visits to be tracked with GPS location verification at clock in/out. You will need a smart phone to access the timekeeping application once you arrive at the client's location.
Driving
- Have a valid license and car with current insurance. Some clients will need transportation.
Documenting
- Must create and enter specific progress notes after each shift. Also, a progress report is due once per quarter.
Scheduling
- Each DSP has a set schedule and must be able to maintain this with clients.
Reliability
- Because Family Relief Resources is not a facility, we need reliable employees who can work independently, show up for their shifts, and be excellent communicators with their client's families and with our agency.

- Helping those with disabilities requires a person who can be patient in all situations, show kindness, and have a positive attitude toward the client and other members of the client's family even in challenging situations.
Flexibility
- Being able to adjust to different types of personalities and disabilities from day to day.
Minimum Job Requirements:
18 years or older
Pass a Criminal History Check.
Work a minimum of 15 hours per month.
Valid driver's license and insurance.
CPR and First Aid Certificate (within 3 months of hire)
State required online training courses (pre-hire and within 3 months of hire)
Continuing Education (24 hours per year)
Basic knowledge of technology, including the ability to use a smartphone for timekeeping, submitting daily and quarterly shift reports, communication with office and client family, and additional requirements as needed.
Has direct experience working with children and/or adults with varying levels of support needs (Including experience with intellectual and/or behavioral disabilities).
Understand requirements of maintaining confidentiality and safeguarding individual information.
Be literate and capable of understanding written and oral orders.
Be able to communicate with individuals, health care providers, case managers, and appropriate others.
Be able to respond to emergency situations when services are being delivered.
