In this demanding but highly rewarding role, you will work with our clients to support their physical or mental challenges in their everyday lives so they can remain in their homes. You will focus on helping them with basic personal hygiene and domestic tasks as needed, according to their requirements and limitations. You will provide compassionate care and assistance and document client progress, either written, electronically or verbally, to help ensure their overall well-being.
Duties and Responsibilities:
Provide basic domestic services (i.e. meal prep, laundry, light housework)
Provide companionship and engage in conversation with the client
Assist with mobility issues, helping client get in and out of bed, wheelchair, etc.
Remind client to take medications
Help with personal hygiene (i.e. brushing teeth, toileting, bathing)
Change bed linens regularly
Assist with all daily activities, based on client's limitations and needs
Provide a caring and positive presence for the client
Act quickly and responsibly in cases of emergency
Requirements and Qualifications:
Minimum of one year of experience working in a home care or healthcare setting
Valid Driver's License is a PLUS, but not required
Willingness to comply with all legal and institutional requirements and guidelines

Schedule: Friday - Saturday 11:00 PM - 9:00AM (Awake) (20 hours)
Essential Duties and Responsibilities:
Provide support and assist people in our services with skills training in all activities of daily living during the day, evening and/or overnight hours (depending on the shift the employee is assigned).
Create opportunities and provide support to people in our services to make decisions to develop their skills and foster independence.
Provide services to people in our services according to DDS, DMH, MassHealth, HUD, DPH, CARF and all applicable regulatory standards, as well as Bridgewell policies and procedures.
Provide support and assist people in our services with all medical, clinical and financial needs.
Transport and accompany people in our services as needed to and from medical, recreational and other activities outside the program.
Maintain daily progress notes, shift checklists, shift assignments and other required documentation according to DDS, DMH, MassHealth, HUD, DPH, CARF and all applicable regulatory standards as well as Bridgewell policies and procedures.
Complete household cleaning duties as assigned.
Conduct all steady care calls as per guidelines (applies to all awake overnight shifts)
Assist people in our services with accessing resources and recreational opportunities in the community.
Administer medications according to MAP regulations, as applicable.
Maintain all mandatory training certifications such as FA/CPR, PABC and MAP (not all all-inclusive list).
Maintain additional mandated certifications and trainings as required by the program and Bridgewell policies and procedures.
Develop positive relationships with guardians, family members and other stakeholders.
Serve as the program's specific Fire Safety Officer or Safety Coordinator or Human Rights Officer.
Comply with Bridgewell Safety Checks Policy.
Attend and participate in program staff meetings and supervision meetings.

Required Education/Experience:

Physical Demands:

Job Summary
Mass General Brigham Home Care provides non-acute, medically necessary skilled care in the home to help "homebound" patients recover from illness or injury and manage exacerbations of chronic disease. Services include skilled nursing care (IV drug administration, injections, tube feeding, wound care, care plan education), physical therapy, occupational therapy, speech language pathology, medical social services, and as needed medical supplies.
Qualifications
Perform paraprofessional home health aide services by following a written plan of care. Work is performed under the supervision of a primary nurse or therapist. HHA will provide personalcare and related activities of daily living services for patients in their homes, skilled nursing facilities and assisted living facilities. Reports and documentation will be prepared, and regulatory and agency guidelines will be followed.
