Position Summary: The Nocturnist Advanced Practice Provider (APP) provides care to complex acute, critical and chronically ill patients and manages the plan of care. The APP is experienced and works autonomously, as well as in collaboration with other health care professionals, to diagnose and manage patients' acute and chronic health care needs. APP patient coverage requires regularly scheduled nights, including weekends, holidays and on-call obligations. The nocturnist position is a full-time night commitment with a reduction in total number of hours per month when compared to day team members.
The Newborn and Infant Critical Care Unit (NICCU) is a sophisticated 58-bed unit that provides a complete range of medical and surgical services for neonates and infants with the most complex conditions. Our unit serves as a regional referral center for the western United States as well as an international referral center. Our specialized programs include Fetal & Neonatal Surgery, Extracorporeal Membrane Oxygenation (ECMO), Whole-Body Therapeutic Hypothermia, Congenital Diaphragmatic Hernia, complex surgical specialties, highly specialized ventilatory support & home ventilation program, Newborn Follow-Up, and more.
Job Responsibilities:
Assesses the complex acute, critical and chronically ill patient for urgent and emergent conditions. Uses relevant data to evaluate for physiologic instability. Utilizes the data to formulate a plan of care.
Performs therapeutic interventions to stabilize acute and critical health problems.
Analyzes the indications, contraindications, risk of complications and cost-benefits of therapeutic interventions.
Manages the plan of care through evaluation, modification and documentation according to the patient's response to therapy. Manages changes in condition with a focus on optimizing the patient outcomes.
Facilitates the patient's transitions between and within health care settings.
Serves as an educational resource to the patient/family, nursing staff, and provider team. Acts as a consultant to multidisciplinary health care team members.
Participates in scholarly activities including research and education. Participates in own professional development by maintaining required competencies, identifying learning needs and seeking appropriate educational offerings. Participates in the professional development of others. Acts as a preceptor and mentor to support the development of nurses, advanced practice providers, students, and other health care providers.
Demonstrates a commitment to patient safety by speaking up about safety issues and changing practice to enhance safety. Shares errors or potential errors through system reporting mechanisms. Demonstrates shared accountability for safe practice.
Minimum Qualifications: NICU clinical experience is required.
Education/Licensure/Certifications:
For NNP: Graduate of a NLN accredited graduate program and NCC certification
For PA: Graduate of an ARC-PA accredited graduate program and NCCPA certification
Nurse Practitioner or Physician Associate license in state of California
Drug Enforcement Administration (DEA) furnishing license

• Serve nourishment in accordance with established facility procedures.
• Feed residents who cannot feed themselves.
• Assure that resident's food is accessible and self-help devices are available as needed.
• Prepare residents for therapy, activities, physician visits, religious services, etc., to include transporting residents to/from such activities.
• Keep incontinent residents clean and dry as possible at all times. Change bed linens, diapers and clothing as often as necessary.
• Do not permit residents to smoke in bed.
• Assure that the nurse's call system is attached to the bed and within easy reach at all times when residents are bedfast.
• Answer residents' call lights promptly.
• Check for food in residents' room. Ensure that food is kept in a sealed container.
• Report all complaints to the Charge Nurse.
• Report any resident abuse immediately (ie., harsh/abusive language, unnecessary roughness, etc.)

• Comb and brush resident's hair daily. Shampoo hair on shower days.
• Clip and trim fingernails.

• Observe and report the presence of reddened areas on bony prominences.
• Keep drainage bag off the floor when resident is in bed or in wheelchair.
• Apply Vaseline or A & D ointment to dry skin in accordance with established policies.
• Move and turn bedfast residents at least every two hours and record such information on CNA notes.
• Observe disoriented and comatose residents and notify Charge Nurse immediately.
• Apply postural support properly. Check with Charge Nurse for type of restraint to be used.
• Apply hand roll or splint to residents with contractures as outlined within the facility's established policies and procedures.

• Chart required information every shift.
• Complete Intake and Output documentation on all residents with orders before the end of each shift.

Assist residents with activities of daily living including bathing, dressing, grooming, toileting, incontinence care, transferring, ambulation according to their individualized service plans. Participate in service plan meetings for specified residents.
Participate in housekeeping/laundry tasks to provide a clean and safe environment for residents/staff to work and live in; this may include: bed making; laundry of residents personal items, towels and flat linens; maintaining cleanliness of surfaces and floors in individual apartments and common areas.
Assist with dining services: plating of food in dining room, serving residents, cutting food, assisting with feeding by offering encouragement, cueing, providing hand over hand guidance and/or feeding.
Participate in life enrichment programming to include gathering of residents; escorts to/from, supervision and leading of programs; and in family and social activities.
Report and record ANY changes in resident condition (physical, psychological, behavioral, social) to nurse on duty.
Document resident status per community policy.
Apply principles of infection control, including standard precautions, hand hygiene, personal protective equipment, cleaning and disinfection, respiratory hygiene, waste disposal.
Monitor resident environment to ensure all safety standards are met or exceeded.
Participate in the admission of new residents.
Promptly respond to all pendants, call lights and pull cords ensuring highest level of hospitality and customer service. Anticipate resident needs as much as possible.
Routinely make rounds of resident care areas to ensure resident wellbeing.
Provide emergency response within scope of practice
Follow through with all assignments completely, thoroughly and timely. Work collaboratively with team members to ensure success of community.
