Responsibilities of our Home Health Aide include:
Promote a safe environment and maintain unobstructed pathways while providing care. Assist with ambulation, transfers and/or range of motion exercises. Follow the assignment sheet/service plan performing only tasks that are assigned. Recognize, report, and record/document changes in patient/client condition and safety to supervisor. Assist, perform, and train patient/client and family in activities of daily living such as bathing, grooming, nail and skin care, oral hygiene, toileting, etc.

Position Summary
Helps Resident by providing comfort. Supports daily living needs of the resident such as assisting with personal hygiene and vital sign monitoring. Able to effectively interact with residents, resident families, and team members.
Enhances community reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to those we serve.
Provides resident comfort by maintaining resident stability by checking vital signs and weight; responding to resident requests; reporting observations of the resident to nursing supervisor.
Provides for activities of daily living by assisting with serving meals and in-between meals snacks and refreshments.
Provides residents' personal hygiene by giving bedpans, urinals, baths, shampoos, and shaves; assisting with travel to the bathroom; helping with showers and baths.
Education and/or Experience
High school graduate or GED.
Successful completion of the Direct Care Worker online test through the Department of Human Services.
