What you will do
Provides primary care for the trauma population in the inpatient and outpatient setting. Ensures that care delivered to patients is provided according to the Stormont-Vail HealthCare mission, strategic plans, and all internal/external regulations, policies and procedures. Practices within the Kansas Nurse Practice Act and within the guidelines of management protocols. Supports the established care models of Stormont Vail Regional Health Center. Evaluates patients in the outpatient clinic setting, monitors problem development and provides appropriate referrals for follow-up and problem resolution.
Functions independently and collaboratively with the physician and all disciplines to provide continuity of health care services for individuals, families, and/or groups. Evaluates the health status, acute episodes of illness, and health care needs of individuals through emphasis on history, physical exam, and psychosocial assessment in collaboration with other health care teams. May perform APP Lead duties as assigned including but not limited to schedule maintenance, coordinating coverage and PTO requests, monitoring assignments, assisting with performance evaluations and other clinical practice duties.
Analyzes data to identify health/illness problems, the human responses to actual or potential health/illness problems, variations from the norm in diagnosis, emergent problems, and complications. Communicates new or altered findings to the trauma surgeon.
Designs and evaluates individualized treatment plan. Coordinates care for continuity of care across the health care continuum. Collaborates with trauma services team to improve care and achieve verification. Under the supervision of the trauma surgeon, writes and effectively communicates orders in accordance with management protocols.
Attends trauma service rounds, participates in patient evaluation and care planning, reviews laboratory studies, vital signs and x-rays. Intervenes as determined appropriate in collaboration with the trauma team and within management protocols standards.
Incumbents will maintain bedside nursing skills by providing three flexible bedside nursing shifts per month and may be designated to work at the bedside for further shifts. Maintenance of skills work and assignment at bedside will be determined by leadership.
Dictates discharge summaries with physician co-signing. Provides early identification of discharge needs and ensures that appropriate referrals are made with case management and social services, nursing care team and attending RN.
Consults with other members of the health care team for emergency problems to expedite disposition/resolution.
Facilitates patient/family participation in health care by identifying learning needs and providing education to enhance decision-making. After discussion of the plan of care with the Trauma Surgeon, provides information to patients and their families regarding diagnosis, plan of care, expectations, and prognosis.
Assists staff in the coordination of a plan of care for the Trauma Services patients.
Refers Trauma Services patients to in-house support services and/or community services as needed.
Performs expanded role in the outpatient setting including suture, staple removal, wound care and other therapies in accordance with management protocols.
Provides telephone consultation to patients and/or physicians following discharge when on duty. Communicates with outreach physicians, hospitals, and EMS.
Has prescriptive authority per Kansas Nurse Practice Act and SVH protocols.
Assists in the development of research based clinical care protocols, guidelines, and pathways for trauma services patients. Facilitates changes in clinical practice based upon document research. Monitors and analyzes outcomes based on research driven practice change. Assists staff nurses in participating in research and incorporating the findings into their practice.

6. Provides simple dressing changes which do not require the skills of an RN, but under the supervision of an RN: observing and practicing all infection control/universal precautions.
7. Assisting the client with medications that are ordinarily self-administered and do not require the skills of an RN or LPN, and under the guidelines of the Kansas Home Health Regulations; in a safe and effective manner.
8. Assistance with activities that are directly supportive of skilled therapy services but do not require the skills of a therapist to be safely and effectively performed. Examples of such activities may be routine maintenance exercises, repetitive speech routines.
9. Collects urine, stool, or sputum specimens from the client, observing and practicing all universal precautions, and delivering to the hospital laboratory as directed by the supervising RN. When delivering of biohazardous materials, observes the agency policy and procedure in safe transport of all materials.
10. Reports changes in the client condition, client needs and daily or caregiver situations to the supervising RN verbally and /or in writing in a timely manner.
11. Attends staff meetings, in-services, case conference meetings so to maximize coordination of services to the client.
12. Completes 12 hours of Home Health Aide inservice hours per year as required by Medicare and the Kansas Home Health Care Regulations.
13. Provided a positive working environment for all co-workers, colleagues, and visitors to the department through two-way communication, good listening techniques and consideration of differing opinions or difficult situations. 14. Provides effective communication with client and family/caregivers in the home in ways that improve client morale and contribute toward meeting goals of client discharge from program.
15. Follows the written instructions on all client care per the Plan of Care (485), the aide assignment sheet and any superseding physician orders.

If not a Certified Nursing Assistant (CNA) one of the following criteria must be met:
Enrolled in a professional (BSN/AD) or practical (LPN) nursing education program and have successfully completed the fundamentals in nursing course.
Within the past 3 years, was a professional (BSN/AD) or practical (LPN) nursing licensure candidate who failed to pass the state licensure examination in any state.
Completed an UAP (unlicensed assistive personnel) program in Missouri or another State (that meets current regulatory requirements) within the past 3 years.
Completed a professional (BSN/AD) or practical (LPN) nursing program outside the United States and is awaiting the licensure examination in this country.
Worked as a nurse assistant, emergency medical technician (EMT), or surgical technician within the past 3 years. Some units may require bilingual skills.

Our pay ranges are market competitive. The pay range for this job begins at $17.15/hr, but your offer will be determined based on your education and experience.

Position Overview
Plan, coordinate, and assist with programs and activities to provide life-enriching events for residents and the community.
Essential Job Responsibilities
Assist with planning, implementing, and leading activities in the following categories: physical, social, recreational, intellectual/educational, creative, entertainment, religious, service/volunteering, and special requests of the residents.
Promote activities in a way that piques residents' interest.
Chaperone activities outside the facility.
Become acquainted with each resident and their activity preferences.
Assist with decoration of the facility for special events, seasons, and holidays.
Invite and/or remind residents to participate in planned activities, assisting them to and from activities.
Drive the company vehicle for resident activities and appointments, as needed.
Provide input for the resident assistance plan and volunteer plan.
Ensure efficient operation of activity department.
Identify and requisition needed supplies for planned activities.
Report on-the-job injuries to the supervisor before the end of the work shift on the day the injury occurs
Maintain confidentiality of verbal and written information pertaining to resident, community operations and personnel.
Other duties, as assigned.
Education, Experience and Other Requirements
High School diploma or GED
Must be at least 18 years of age
Knowledge, Skills, and Abilities Required
Promote teamwork in providing services to residents.
Demonstrated organizational and communication skills
Microsoft Office programs
Preferred Qualifications
Valid driver's license, CDL preferred.
Clean driving record and willingness to drive company vehicle to transport residents
