This facility operates 24/7/365; Response is required during an emergency activation. 2. The BHP consults with medical, contract staff, and other healthcare professionals to conduct and provide education and training on continuity of care, treatment, monitoring and management of patients with co-occurring diagnoses. REQUIRED EDUCATION AND EXPERIENCE Degree issuing institution and or program must be accredited by an entity recognized by the U.S. Department of Education * Master's Degree in Social Work from a program accredited by the Council on Social Work Education (CSWE). * Minimum of one-year experience as a Licensed Clinical Social Worker, at the independent level as recognized in the state licensed, whereby clinical supervision is not required. Minimum of 1 year of experience in a home health or hospice setting. Minimum of 1 year of full-time experience in direct client care in an institutional setting (hospital or nursing facility). Must have the ability to recognize the needs of people, to maintain good relationships with people, and have the capacity to make meaningful observations and communicate them to the supervisor.
