The Direct Care staff employed by CHD are hired under a specific residential program but can work within the other Congregate Care Programs as required by their direct supervisor. The behavioral health profiles/histories of the youth being referred to the congregate care settings are complex and require 24/7 supervision. Therefore the direct care staff assists in maintaining a structured and therapeutic environment that builds on the youths strengths to build self-regulation skills; responds to the daily care and treatment needs of all youth in a manner that is respectful of and responsive to diversity across all human characteristics. The direct care staff deliver behavioral health treatment that is in line with the youth's treatment plans and effective at all times. The direct care staff is a professional who actively carries out the philosophy of the program(s) and agency while maintaining a safe, secure, and therapeutic environment. They will guide the youth in a strength-based manner and will play an instrumental role in child's stay in the program. The direct care staff will work in collaboration with the program, the youth, his/her family, DCF, and other supporting agencies. The direct care staff report to the Program Manager or designee and receive supervision from their direct supervisor. Part Time/Relief Staff: Part Time must work 20 hours a week minimum. Relief staff are often asked to pick up any of the three main shifts (7-3pm, 3-11pm, or 11pm-7am) and are expected to perform those specific job functions at all times just like a full time staff is i.e. overnights are expected to do regular bed checks and remain awake and alert at all times. As a Relief Staff, you are working on a relief or variable basis with no guarantee of hours or an obligation to accept all hours offered. Pay Rate: $20 an hour Open Shift: Saturday 4pm-8pm (4h) Key Responsibilities Engage with participants in a respectful and empowering manner to promote self-esteem, independence, and personal growth. Assist participants with daily living tasks such as personal care, meal preparation, shopping, and household maintenance. Support participants in accessing community resources, including transportation, education, social activities, and healthcare services. Teach and encourage self-advocacy skills while serving as a liaison with families, providers, and external parties. Provide financial support through budgeting, money management, and accurate documentation of expenditures. Monitor participant health and well-being, assist with medication administration, schedule appointments, and respond to emergencies. Maintain accurate, timely documentation including logs, data records, incident reports, and use of agency systems. Promote safety by identifying and addressing hazards, assisting with emergency drills, and following agency safety procedures. Demonstrate flexibility in scheduling, provide shift coverage as needed, and actively participate in team collaboration. The Primary Care Nurse Practitioner (APRN) or Physician Assistant (PA) Resident functions as a member of a medical team and applies to the treatment of patient's the medical and related skills acquired through the training and/or experience required for this position. The resident will receive continuous mentorship while providing direct clinical care to our diverse patient population as well as engage in formal weekly educational didactics. APRN or PA residents will leave our residency program with enhanced clinical skills and tools to make a lasting impact on community health. This position will provide services within one or more Wheeler program(s), and the relative concentration between and within programs may change with the needs of the clinic. Manages prescribed medication, performs physical health examinations, conducts preventive screening and care, develops and presents clear clinical impressions and recommendations that are specific and attainable. Develops and documents clear clinical formulations and medical treatment plans. Prescribes therapeutic interventions appropriate to identified problems. Administers drugs and biologicals as required by the Clinic. Demonstrates a basic knowledge of physical health concerns and effective medical interventions. Triages clinical emergencies and implements safety and consultation protocols. Utilizes evidence based and best medical practice. Monitors patients' self-care management and self-care skills and provides prevention, wellness, and pertinent medical education. Provides requisitions for clinical studies, diagnostic tests, and laboratory studies. Evaluates and interprets results to identify and assess patient's clinical problems and health care needs. Demonstrates knowledge of internal and external levels of care, referral processes and resources. Makes clinically appropriate referrals for consultation with specialists, subspecialist, and allied health professionals as indicated. Ensures all patients' medical records are completed timely and in accordance with organization policy and that all charting is comprehensive and legible. Works with care coordination team in designing, monitoring and implementing care plans. Communicates referrals and collaborates on development of services planning and implementation. Maintains consistent communication with community providers, local and state agencies, and other allied professionals and services mutually involved in client care. Represents the Clinic in external and internal committee meetings as assigned. Communicates in a professional, positive, effective, and courteous manner with client, family, and all internal and external providers. Meets direct service productivity goals as defined by program. Demonstrates knowledge of program/department/agency goals and target outcomes, and adheres to practice standards and to data collection and reporting protocols. Attends and is prepared for scheduled meetings with administrative supervisor. Attends and is prepared for scheduled clinical meetings and consultation with supervising physician. Seeks additional supervision or consultation as needed and follows through with clinical directives. Provides service in the office and/or community settings outside of the office (e.g., homes, schools, hospitals, institutions, other community locations) as defined by the program. Must be able to pass a respirator fit test at time of hire and on an annual basis. Providers will treat HIV, Hepatitis C, and provider gender affirming care within their primary care practice. Support will be provided by the medical director and other provided resources. Provide services on the mobile unit, as required. Continues to develop knowledge and understanding about the history, traditions, values, family systems, and artistic expression of groups served as well as uses appropriate methodological approaches, skills, and techniques that reflect an understanding of culture.
