Job Overview
We are seeking an APP to provide office-based care and see patients independently alongside collaborating physicians and APPs. Our APPs mostly work within Centers of Excellence built around specific urologic conditions. Office-based procedures can be taught if desired. Most common diagnoses would include: kidney stones, male voiding dysfunction, female voiding dysfunction, elevated PSA/prostate cancer, erectile dysfunction, etc.
Duties and Responsibilities
Demonstrate a high degree of general medical and urological knowledge and exercise independent judgment as appropriate
Assess the patient's medical condition by completion of a urological examination
Document appropriately and communicate with the appropriate physician in a timely manner as required, including, but not limited to: evaluations, treatment plans, progress reports, treatment notes, and other reports as requested
Order and evaluate laboratory, radiological and diagnostic tests
Prescribe, monitor, and adjust medications and devices based on the patient's medical condition
Develop and perform treatment plans in accordance with established protocols
Order physical therapy, occupational therapy and/or home health care as appropriate
Perform procedures in office setting (cystoscopy, prostate biopsy, etc.) as trained

The Department of Medicine has developed a Comprehensive Sickle Cell Program in collaboration with the Departments of Physical Medicine and Rehabilitation and Psychiatry. This program is supported by EVMS and Sentara Health. The goal of this program is to provide top-notch outpatient and inpatient clinical care to patients with sickle cell disease. We are looking for Advanced Practice Providers (APP), a nurse practitioner (NP) or a physician assistant (PA), who will function as a clinical provider in the Division of General Internal Medicine, Department of Medicine. The APP will participate in all aspects of patient care and will participate in the supporting roles of consultant and educator as indicated. The clinical practice will mostly focus on clinical care of patients with Sickle Cell Disease in the hospital and in the outpatient clinic under supervision of clinical faculty. The clinical practice may also include provision of primary care to all types of patients.
Responsibilities
Independently assess, diagnose, & develop treatment plans for patients with sickle cell disease as well as patients in the area of Primary Care in accordance with protocols and standards of care.
Promote a schedule of health promotion examinations and interventions in our patient population
Obtain medical histories, perform physical examinations, and prescribe, dispense or deliver medications under a collaborative practice agreement.
Differentiate between normal and abnormal findings and orders appropriate diagnostic studies.
Provide anticipatory guidance and counseling to patients/families.
Document patient assessment and plan of care in the medical record and generate appropriate outgoing correspondence.
Be familiar with documentation requirements to insure proper billing.
Initiate and coordinate referrals to other health care team members or agencies.
Serve as a consultative resource to nursing staff.
Participate in the development, utilization and evaluation of resources to meet patient/family educational needs.
Provide education to external agencies, community and other health professionals.
Participate in development and implementation of divisional and departmental quality assessment/improvement/research programs and processes.
Monitor task module of the electronic health record to insure timely response.
Responsible for administrative activities and special projects as assigned.
Contribute toward the achievement of divisional and departmental goals.
Participate and contribute to activities that enhance clinical expertise and nursing knowledge.

Report non-compliance with policies, procedures, regulations, or breaches in confidentiality to appropriate personnel.
Protect residents from abuse and cooperate with all investigations.
Report any allegations of abuse, neglect, misappropriation of property, exploitation, or mistreatment of residents to supervisor and/or administrator.
Report any retaliation or discrimination to the Human Resource Director and the Compliance Officer.
Fulfill individual responsibility to report any suspicion of a crime to law enforcement and the state agency.

Primary Responsibilities
⢠Directly assist residents with daily tasks, maintaining communication with the Resident Care Director regarding care provision and resident condition changes.
⢠Aid residents with mobility, utilizing proper techniques and equipment to prevent accidents.
⢠Make routine resident care rounds to ensure the needs of each resident are being met in accordance with their individualized care plan.
⢠Assist with lifts, moves, and transport, using proper body mechanics or lifting devices for accident prevention.
⢠Performs care that maintains each resident's skin integrity to prevent pressure ulcers, skin tears and other damage by changing incontinent residents.
⢠Document daily care activities in accordance with company policy and regulatory standards.
⢠Report any resident issues or concerns promptly to the appropriate supervisor.
⢠Respond promptly and courteously to inquiries from residents, visitors, and staff, adhering to established policies.
⢠Familiarize oneself with residents' individualized care plans and provide support accordingly.

The RCA must be capable of lifting, stooping, carrying, turning, and assisting high-risk residents, as well as operating medication carts. They must also be willing and able to work a flexible schedule, maintaining continuous movement throughout work hours. The ability to lift and/or carry 50 to 75 pounds is necessary.
