The individual will meet with families and caregivers to explain care delivery, and will collaborate with discharge planners and physicians for the entry of patients into a facility.

* Provide educational in-services and other educational opportunities to the community and providers in assigned territory annually at minimum.
* Responsible for census development and growth of long-term care inpatient care.
