Job Summary: Provide care to pets, direct care and maintenance of kennels, hospital wards, and surroundings. Must treat animals with care and respect, work well with dogs and cats. Keeping pets and kennels clean, monitor, and manage pet care responsibilities daily.
Education and Experience: - High school diploma or equivalent.
Dependable attendance is required. - Any allergies to animals must be controllable through medication. - Must be able to lift 40 pounds. - Must be willing to work flexible hours, and a split-shift on the weekends and holidays. - This position requires the ability to walk, bend, stand and reach constantly during a scheduled shift of 8, 10, or 12 hours as assigned. - Visual acuity sufficient to maintain accurate records, recognize people and understand written directions. - Ability to speak and hear sufficiently to understand, give information in person and over the telephone.

Nursing Care: - Compassionate nursing care is the top priority for all patients. - Keep kennels clean, orderly and free of odors. - Feed patients as scheduled. - Exercise (walk) patients. - Ensure the comfort and cleanliness of hospitalized or boarding pets. - Bathe, trim nails, clean ears, and brush pets.
Housekeeping and Maintenance of Hospital: - Stock supplies as needed. - Keep the exterior of the hospital clean and orderly. - Janitorial care of the hospital facility - Make certain entire hospital, including treatment, surgery, kennels and runs are kept extremely clean through the day. - Advise management of any conditions that are unsafe or hazardous.

Reports To: Clinical Director
The Care Coordination Specialist plays a key role in the Collaborative Care Management (CoCM) model within our behavioral health practice. This role serves as the first point of contact for patients referred into behavioral health services. The Specialist is responsible for obtaining patient consent for participation in Collaborative Care, ensuring seamless handoff of referrals to Behavioral Health Care Managers, and supporting the care team by coordinating services that address patients behavioral and psychosocial needs.
Key Responsibilities
Serve as the initial liaison for patients referred to behavioral health services.
Conduct informed consent discussions with patients, ensuring understanding of the Collaborative Care Model.
Collect and document patient consents in compliance with organizational and regulatory standards.
Facilitate warm handoffs and timely referrals to Behavioral Health Care Managers (BHCMs).
Maintain accurate and thorough documentation in the electronic health record (EHR).
Coordinate with medical providers, BHCMs, and psychiatric consultants to ensure continuity of care.
Monitor referral completion and follow up with patients as needed.
Provide patients with information, resources, and support to improve access to care.
Participate in care team meetings and communicate relevant updates to the behavioral health team.
Maintain compliance with HIPAA and ethical standards of professional practice.
Qualifications
Education: Masters Degree in Social Work (Non-Clinical; Administrative)
Licensure (Required): LMSW
Strong interpersonal and communication skills with the ability to engage patients sensitively and effectively.
Knowledge of behavioral health and medical systems of care.
Detail-oriented with strong organizational and documentation skills.
Ability to work collaboratively in a multidisciplinary team environment.
Preferred Experience
Prior experience in care coordination, patient navigation, or case management.
Familiarity with the Collaborative Care Model (CoCM) and/or integrated behavioral health.
Experience in oncology, chronic illness, or primary care settings.
Bilingual (Spanish/English) strongly preferred.
Work Environment & Schedule
Full-time, in-person role based in the Anniston, AL area.
