Job Title & Role Description:
The Care Specialist is a key member of the team responsible for delivering chronic care management to high-complexity patients. The Care Specialist primarily works in patients' homes and communities (90% of the time) and engages in virtual care (10% of the time). This role involves direct outreach to patients, guiding them through the enrollment process, facilitating virtual appointments with providers, and ensuring that patients adhere to care plans. By building relationships, coordinating care, and providing education, the Care Specialist plays a critical role in promoting health, preventing unnecessary hospitalizations, and improving patient outcomes.
Skills Required:
Minimum of 2 years of relevant work experience (e.g., Community Health Worker, Peer Support Specialist, Medical Assistant, etc.).
High school diploma or GED required.
A valid drivers license, auto liability insurance, and reliable transportation to travel within the assigned territory.
Experience in chronic care management or working with chronically ill/elderly patients.
Technologically proficient with basic computer skills (typing, using EMR systems).
Experience with motivational interviewing, trauma-informed care, and care coordination.
Strong interpersonal communication skills with the ability to engage patients and team members effectively.
Ability to prioritize tasks, manage schedules, and work independently in an unstructured environment.
Multi-lingual skills are a plus but not required.

Your Duties:
Typically, pet-sitting visits and walks occur within these time windows:
Mornings: 7 am - 9:30 am

dog walking, anyway? If you live in NYC, then you know that these small boxes we call "apartments" can have a dog feeling cooped up, bored, and desperate for fresh air! For our clients who work long hours - that's almost everyone in this town - we provide a mid-day outing for their pup, either alone or with 1-2 buddies, so they can get outside, get some exercise, and do their business. We also provide support for puppies, cat sitting services, and year-round care for all NYC Pet Pawrents!
I've seen those guys getting dragged along by 10 dogs down the sidewalk.
Is that what dog walkers do? Right, some dog walkers work for companies that do what's called "pack" or "group" walks. These are often the same jobs that pay a flat hourly wage, regardless of how many dogs are being walked at a time or in a day, and regardless of how far away the dogs are from each other. By contrast, we walk a dog with one or two of his buddies who live nearby. This provides socialization for our pups, but in a safe, controlled environment where all our dogs can be watched carefully, attended to as individuals, and our staff are not being dragged along by packs of dogs all day. Ours is a very different approach, much closer to childcare.
REQUIREMENTS
First and foremost you must love dogs & cats. (We assume that you do, or you wouldn't be reading this!) ... beyond that,
we look for someone who can master the THREE ASPECTS OF THIS JOB:
1. Be great with the PETS! Lots of people say they love animals, but it takes a special kind dedication to care for animals as your JOB. Neither the snow, nor rain, nor hissing scaredy-cat nor gloom of night.. nothing will keep you from getting to the pets in your care! Are you the kind of person who stops to pet all the dogs, sometimes forgetting to say hello to the human walking them? WE WANT TO MEET YOU!2. Be comfortable communicating with PEOPLE! We do lots of emailing and texting among our staff and clients, so we are looking for a new hire who can get back to us swiftly and professionally. Goes without saying, but this business in built on TRUST, and communication is the key to that!3. Be adept at using TECHNOLOGY! You'll use our scheduling app which keeps track of your jobs and earnings. Applicants need to have a signal-enabled smartphone (a phone that can function even with no available wifi) with enough data to handle the amount of pictures, emailing and texting we do each day. Your basic iPhone 8 plus with 128g would be about the minimum reqs there. Any smartphone with a robust data plan will work!

Responsibilities:
Ensure that each guest is treated with respect and dignity
Ensure that all guests are receiving appropriate amounts of food and water, clean and sanitize food/water bowls on a daily basis.
Clean up dog poop, urination and any vomit several times throughout your shift
Walk dogs on leash to and from outside exercise areas.
Engage in play activities with pets including: fetch, exercise walks, chase, agility, follow the leader, recall and basic obedience games, etc.
Supervise activities of multiple dogs throughout the day
Monitor closely for any potential conflicts or escalations and redirect or remove one or more dogs from the situation as needed
Maintain a sanitary and healthy environment by following our standards of cleaning and disinfecting rooms
Monitor guest's health by daily charting of eating and elimination habits. Dispose of and clean up any stool and urine.
Monitor guest's reaction to playtimes and general demeanor by charting playtime interactions
Follow all standards of safety with regards to handling guests, particularly aggressive guests and escape artists
Pay specific attention to special handling notes, (Meds Person to Feed, Aggressive, Separate for Feeding, etc.)
Immediately alert front staff and resort manager when a guest is ill or exhibiting odd behavior - follow-up by filling out illness form and turning it into a shift lead/manager.
Operate washer and dryer to clean pet belongings, towels, etc. Ensure belongings are returned to the appropriate pet.
Ensure the facility meets or exceeds our expectations with regards to cleanliness and safety. Clean and sanitize all pet enclosures, exercise yards, and common areas.Mix and use disinfectants and other chemicals safely as instructed by the supervisor.
Ensure that the property is maintained (lawn care, snow removal, tree maintenance)
Alert manager when levels of inventory are low in the following areas:
Pet foods
Cleaning products for kenneling and lobby areas
Help maintain the cleanliness of the property by picking up and disposing of trash or debris. Keep aisles, drains, carts, shelves, sinks, floors, furniture, counters and glass clean, sanitary and free of clutter/debris. Empty and sanitize trash cans and receptacles.
Alert manager of any safety or operational issues ie: broken guillotine doors, burnt-out lights, not enough salt for snowy areas, etc.
Keep informed of important notices, meetings, etc. by consulting the "Guest Services Command Center"
Consult with other departments

DETAILS:
20-30 hours per week
Shifts are 9/2pm, 10-3pm or 11-4pm EVERY DAY Monday-Friday and one weekend per month
Areas: Monroe County
You will drive from house to house according to company schedule providing dog walks, puppy care, cat care, and pet sits for the pets in each household
What We are Looking For:
Must have reliable transportation, a valid drivers license, and insurance
Must have a smart phone (and able to use our apps)
Must be over 18
Must be able to handle large dogs that pull on leash
Must be willing to commit for at least 6 months
