Position Overview
Reporting to the Manager of Pharmacy, the Pharmacist provides services in accordance with established departmental policies and procedures, professional regulations and standards and Provincial and Federal statutes. Principle functions include drug procurement, drug distribution, and clinical pharmacy activities.
The position functions in a manner that is consistent with the mission, vision and values; and the policies of Southern Health-Santé Sud.
Experience
Previous experience in a hospital pharmacy setting is preferred.
Previous knowledgeable and experience with sterile product preparation is required.
Basic computer experience is required including typing skills (minimum 40wpm).
Education (Degree/Diploma/Certificate)
Completion of a Hospital Pharmacy Residency is an asset.
Certification/Licensure/Registration
Current practicing membership or eligibility for registration and membership with the College of Pharmacist of Manitoba (CPhM).
Membership in the Canadian Society of Hospital Pharmacy (CSHP) is an asset.

POSITION SUMMARY:
Provides pharmaceutical services to the patients of Family Health Center and the community utilizing the highest level of professional ethics and standards. Full-Time, Part-Time, and as needed positions available.
DUTIES AND RESPONSIBILITIES:
Accepts, validates, interprets and processes prescription orders.
Initiates and completes calls with physicians regarding appropriate drug therapy.
Conducts Drug Utilization Review (DUR) and provides necessary clinical interventions.
Provides physician education and information/counseling; reconciles drug therapy management issues.
Responsible for Formulary Management - including Therapeutic Interchange or Substitution, and Coverage Review Programs (e.g. Prior Authorization).

The Pharmacist is a professional member of the health care team responsible for "Assuring Safe Medication Use". Dispenses medication in response to a physician's prescription exercising. Professional judgment in accordance with established policies, procedures, standards, and government regulations to ensure appropriate drug therapy for optimal patient care. The Pharmacist counsels, assists, and educates physicians, nurses, and the community on appropriate medication use, doses, side effects, and precautions. Prepares large volume parenterals, IV piggybacks, total parenteral nutrition, chemotherapeutic and antineoplastic agents, IV push medications, irrigations, patient controlled analgesics, ophthalmic preparations, and enteral feedings.
* Assists with overseeing the work of Unit dose and IV Additive Technicians the in performance of their regular duties. Ensures that all drugs and solutions prepared by technicians are accurate and prepared under proper conditions. Checks and verifies accuracy of medications in carts; releases carts for delivery to patient care areas.
* Prepares and checks stock drugs for nursing units and other departments. Monitors pharmacy and nursing unit medications on a regular basis in order to see that all drugs are current and in adequate supply.
* May provide pharmacokinetic evaluation and dosing of patients on specific medications and monitors patient progress. Evaluates and acts on laboratory data and patient response.

The Department of Pharmacy at SUNY Downstate Health Sciences University is seeking an hourly/per diem Outpatient Retail Staff Pharmacist. The Staff Pharmacist reports to the Associate Director of Retail Pharmacy Services and prepares, compounds and dispenses medications for outpatient and ambulatory care use as well as dispenses retail and specialty pharmacy related items. Additionally, the successful candidate will:
* Provide information to health care practitioners to promote the rational use of medications.
