Develops and maintains competency in health-system pharmacy practice, including pharmacotherapy, patient care, drug information, and practice management. Plans, executes, and monitors pharmaceutical care plans. Performs clinical responsibilities such as pharmacotherapy consultations, pharmacokinetics, and TPN initiation/monitoring, as applicable. Evaluates and responds to clinical alerts (e.g., IV to PO conversions, renal dosing protocols). Investigates and reports adverse drug reactions and medication errors, ensuring proper documentation. Participates in multidisciplinary rounds when applicable to site of practice. Maintains punctual and regular attendance in compliance with Baptist Health and applicable regulations (ADA, FMLA). Complies with all Baptist Health policies and procedures. Promotes open communication with patients, families, visitors, physicians, and team members to ensure a positive experience.
