Dispensing Activities:

Provide pharmacist oversight necessary to check medication duplication, dosage, and drug interactions, as well as drug-disease interactions. Establish a formulary in accordance with the requirements of the New Hampshire Veterans Home's Medical Director. Procure and maintain adequate inventory levels for all medication and ensuring their storage under the proper conditions.
Inventory levels shall be determined by the contracted Pharmacist and New Hampshire Veterans Home staff.
Medications shall be ordered as necessary through a combination of the Department of Veterans Affairs pharmaceutical distributor and the approved State of New Hampshire contract through Minnesota Multi-state Contracting Alliance for Pharmacy (MMCAP), or from any other source as directed by Officials of the State of New Hampshire.
All pharmaceuticals ordered are the sole property of the State of New Hampshire and are to be used for the New Hampshire Veterans Home and may not be ordered for any other purpose.
Ensure that controlled substance inventories and records adhere to State and DEA regulations.
Maintain a night cabinet for off-hour drug acquisition by nursing. Size and stock shall be adequate to meet the needs of a 250-bed long-term care facility. Screen all off-hour orders immediately when the pharmacy re-opens.
Provide pharmacy resident profiles to include name, age, medical record number, allergies, code status, medical diagnosis and current medications.
Generate monthly medication administration records and as necessary for new admissions and return from hospital stays. Medication Administration Record (MAR) sheets shall include all special instructions for the proper administration of the prescribed medications. Proof of use sheet for controlled substance shall be provided for each prescription.
Provide Leave of Absence medications as required per New Hampshire Veterans Home policy.
Clinical Pharmacy Services
Provide timely pharmacy interventions to physicians and nurses.
Provide monthly chart reviews by the pharmacist using Omnibus Budget Reconciliation Act of 1990 (OBRA) Program and Health Care Facilities Guidelines criteria and standards applicable and customary for long-term care. A monthly review of each resident's drug regimen, listing incompatibilities, interactions and making appropriate suggestions on lab work to be done, any potential drug/medication problems, etc. The monthly review, in compliance with OBRA and Health Care Facilities Guidelines, shall be performed by a registered pharmacist on-site and the results of each review shall be provided to the Resident Care Services Director.

The CMOP program provides prescription mailings for veterans and other patients. Services being sought will be utilized to assist in supporting CMOP's mission by preparing prescriptions and packages in an accurate, timely, and cost-effective manner without disruption in services or production.
The Candidate
ProSidian seeks a Registered Pharmacist to support engagement with The Veteran's Affairs Consolidated Mail Outpatient Pharmacy (CMOP). This position serves as a key member of the facility leadership team providing services that include, but are not limited to, pharmaceutical verification of prescription orders of legend, over the counter (OTC), medical-surgical (Med/Surg), and controlled substances to patients requiring care. In addition, the candidate shall provide patient consultation, quality improvement, and supply control duties.
Furthermore, the candidate shall direct the delivery of pharmacy services, efficient and effective drug distribution, and education to improve patient quality of life, minimize therapeutic risk, and enhance clinical outcomes. The candidate's leadership attributes in Pharmacy management shall ensure the delivery and attainment of patient care quality metrics, evaluate the performance of Pharmacy Technicians duties, and drive client satisfaction outcomes.

Job Summary
The MA State Office of Pharmacy (MSOPS) Pharmacy Distribution Center (PDC) provides medications via central fill in unit dose or blister card form for DDS, DPH, DMH, DOC, and HOC facilities throughout the Commonwealth of Massachusetts. This position reports to the Director of Pharmacy or their designee. The primary objective of this position is to review and interpret physician orders, accurately dispense medications, support the onsite pharmacy technicians, works effectively under the direct supervision of the Director of Pharmacy and/or their designee to meet the drug needs of the MSOPS patient population.
Schedule
40 hours/week.
Rotating between day and evening shift.
Weekend rotation required.
Responsibilities
Checks unit dose medication for appropriate drug, dose, quantity, and packaging integrity.
Supervise and direct pharmacy support personnel as assigned by the Director of Pharmacy.
Facilitates availability of first doses and specialty items; maintains adequate medication supplies.
Completes monthly controlled substances audits and quality assessment rounds.
Maintains and enforces the documentation and security of narcotic medications.
Review medication orders for therapeutic appropriateness; Reviews transcription of medication orders.
Updates medications on administrative records.
Provides pharmacokinetic monitoring of patients receiving specific drugs and ensures that serum concentrations are drawn; monitors parental nutrition.
Observes for adverse drug reactions; documents all medication incidents within the reporting module.
Provides medication counseling to patients; provides drug information to physicians, nurses, patients and other health care professionals.
Documents interventions with health care personnel related to drug therapy.
Performs drug utilization evaluations on medications; oversees activities of medication administration nurses; participates in patient care rounds.
Provides in-service education to hospital health care professionals.
Repackages bulk medications in unit-dose packages safely and accurately per established procedures using appropriate techniques.
Demonstrates competency in automated dispensing equipment and adheres to established processes and procedures.
Maintains security and confidentiality of passwords.

Qualifications
Bachelor of Science in Pharmacy (BS Pharm) or Doctor of Pharmacy (PharmD) degree from an ACPE-accredited school of pharmacy or equivalent.
Licensed, “good standing” pharmacist in the state of Massachusetts.
Hospital, long term care, or retail experience preferred.
IV experience preferred.
Informatics and medication safety experience preferred.
Ability to work well in a team.

Responsibilities

Pharmacists who are licensed but have been subjected to disciplinary action of any type by their licensing authority within the preceding five years, must be referred to CMOP management before proceeding with the hiring action.
Must be licensed for controlled substances, with qualifications to practice in any states.
