Position OverviewUnited Health Services (UHS) is seeking a dedicated Pharmacist to join our Transitions of Care Pharmacy team. The Pharmacist will ensure the safe and accurate administration of medications by verifying the clinical appropriateness of prescribed treatments and overseeing proper delivery methods, thereby promoting patient safety and optimizing therapeutic outcomes. The Pharmacist is responsible for reviewing complex medication regimens, including prior to admission medications and discharge medications. The Pharmacist will work to ensure a smooth transition from hospital to home, including addressing adherence barriers such as medication affordability and transportation barriers.
