Hours: Pharmacy hours are Monday-Friday: 8:00am-5:00pm, no nights, weekends, or holidays
Primary Responsibilities:
Distributes drugs prescribed by physicians and other health practitioners

The hourly range for this role is $43.17 to $84.95 per hour based on full-time employment. Summary:

b.Reviews drug therapy to ensure appropriate use, dose, dosage form, regimen, route, therapeutic duplication, drug allergies, drug interactions and cost-effective therapy.
c.Reconciles home medications for admitted patients.
d.Discusses medication order clarifications with the prescriber/healthcare team and document any changes in patient and pharmacy records to inform others of medication order changes.
e.Provides individual patient/family counseling on medication regimens, adherence to medication regimens and adverse effects of medication regimens to patients/caregivers prior to discharge of allogeneic SCT patients and other patients deemed appropriate and document in the electronic medical record.
f.Provides pharmacokinetic consultations as required. Kinetic notes are written for every new start of vancomycin or an aminoglycoside and for dose modifications.
g.Prevents, detects monitors, documents and reports adverse drug reactions and medication errors.
h.Reviews chemotherapy orders for hematology, oncology, gyne/onc, and SCT patients receiving chemotherapy inpatient.
i.Available to nursing staff for questions on medication orders.
j.Order verification/in-basket messages for assigned units.
k.PM staffing required, frequency based on departmental need.
l.Changing medications from IV to PO as appropriate.
m.Assist with facilitating chemotherapy desensitizations in the MICU.
Discharge Planning (Inpatient Acute Care):

a.Participate in discharge planning and discuss relevant changes to the home medication list prior to discharge with the team and/or the patient as required.
b.Help facilitate discharge planning for smooth transition to outpatient pharmacies or alternate site of care.
c.Assist in assessing insurance coverage and assist in prior authorization for medications (when necessary) in advance of discharge.

1.The hematology/oncology/SCT pharmacist will:
a.Act as the pharmacy liaison to the Cancer Center.

Job Duties:

Provides drug information to nurses, patients and parents. Monitors drug efficacies, toxicities and patient outcomes through patient clinical status and appropriate laboratory data.
Reviews and prioritizes physician orders. Ensures that medication orders are appropriate according to the patient's clinical status, allergy, drug histories and concomitant drug therapies.
Maintains patient profiles, clinical notes, and narcotics and investigational drug records.
Compounds and dispenses pharmaceuticals. Checks unit dose medication carts and IV admixtures for product, label and volume/amount accuracy.
Takes admitting drug histories when necessary to clarify allergies and home medications.
Assists in cardiopulmonary resuscitations. Participates in drug utilization evaluation.
Works as a team with other members of the department. Supervises pharmacy technicians.
Participates in the performance appraisals of pharmacy employees.

Position Summary:

The ASP-ID pharmacist will be responsible for developing and coordinating innovative clinical pharmacy programs across RUSH sites conducting scholarly and research activities and interdisciplinary teaching. To support the effectiveness of ASP initiatives, the ASP-ID pharmacist will maintain on-site service coverage rounding with inpatient ID team(s). The ASP-ID pharmacist will serve as a key faculty member in postgraduate pharmacy training programs and affiliated colleges of pharmacy. This position will focus their efforts on ASP support at Rush Oak Park hospital and Rush Copley Medical Center.
Essential Clinical Duties and Responsibilities of the ASP-ID Pharmacist:
Develops metrics for performance and maintenance of ASP.
Participates in the provision of pharmaceutical patient care through ASP staffing and clinical rounds.
Participates in restricted/protected antimicrobial approval.
Participate in ASP coordinated education of healthcare professionals across RUSH sites and external partners.

Basic Qualifications

Patient Experience
