Role Description We are currently looking for a Pharmacist at 1st Choice pharmacy Management LLC. The Pharmacist will be responsible for overseeing daily pharmacy operations, ensuring regulatory compliance, managing inventory, providing patient care, and collaborating with the healthcare team. This role requires working onsite in Poway, CA. J OB A NNOUNCEMENT Staff Pharmacist Non- Clinical (Per Diem) Department: Pharmacy Supervised By: Pharmacist in Charge Status: Non-Exempt (Per Diem) Grant: N/A Hours: Monday-Friday, 8:00AM-4:30PM, time may vary to meet the needs of the Clinic GENERAL STATEMENT OF RESPONSIBILITIES : Under the supervision of the Pharmacist in Charge, the Pharmacist is responsible for the planning, evaluation, supervision, coordination, implementation, and administration of comprehensive and effective quality Pharmacy programs (new & existing) at the clinic and will include working with the other site Pharmacist on a regular basis. Duties include providing professional advice/consultation concerning drugs and their administration to patients/staff on a timely basis, the maintenance of a drug formulary, establishing & maintaining excellent patient/staff relations, maintaining licensure, reporting of data, and the billing of revenues from customers. SPECIFIC DUTIES AND RESPONSIBILITIES : 1. Pharmacy Clinical Operations: Accurate and timely administration of prescriptions. Reviews all drug orders for completeness and appropriateness of medication and consults with the prescribing physician immediately to correct any problem areas. When appropriate, reviews patient history, current therapy, diagnosis, allergies, and recent drug history prior to dispensing any medications. Uses generic brands, whenever possible and provides labeled medications to patients and staff, including listing any possible drug interactions. Records prescriptions issued in the medications area of the medical record and oversees all supporting pharmacy data processing and billing information of the patient. 2. Professional Advice/Consultation: Provides professional, courteous, accurate and timely information to requesting patients, medical staff, and clinic personnel immediately, if possible. Responses requiring written replies will be completed and submitted within (2) business days upon request, unless referred to alternate sources. 3. Priority of Patients waiting for Prescriptions: Supplies prescriptions to patients waiting in the lobby after an SIHC visit on a priority basis. Unless restricted by workload, supplies prescriptions within 30 minutes of receipt in the Pharmacy. Prescriptions are announced over an intercom system. For internal control purposes, logs prescription requests in and out of the Pharmacy department. Refills prescriptions via telephone or other requests within 1 hour, not to exceed 4 hours depending on workload. 4. Drug Distribution/Control: Controls ordering, stocking, and distribution of all drugs within the clinic, including all clerical and distributive functions. Stocks and maintains clinic “crash cart” on a monthly basis, or after immediate use. Performs monthly pharmacy and nursing unit inspections and removes all damaged, expired, or unusable drugs from stock. Verifies that proper storage conditions and security are in order upon each inspection. Properly destroys or returns unusable drugs. When required or requested, reviews all therapy for potential drug therapy danger by calculating pharmacokinetic parameters for drug dosing and evaluates fluid and electrolyte requirements and nutritional therapy. With consultation and direction from the Medical Director, develops and maintains a drug formulary and uses generic drugs, whenever possible. Reviews all adverse drug reactions, logs incidents, and files written reports with proper authorities. All transactions requiring special record-keeping and custody shall be recorded accurately and action needed will be implemented immediately. 5. Trauma Activities: Participates in all cardio-pulmonary resuscitation and trauma team activities, as required and in accordance with policies. Responds immediately to “Code Blue” and “Code Trauma” calls and returns immediately when assistance is no longer required. 6. Other clinical Duties. Complies with all federal & state laws, regulations, and practices generally accepted in licensed Pharmacy operations. Perform other clinical duties, as assigned. 7. Pharmacy Administration: Opening/Closing, Billing Responsible for the Pharmacy department's opening/closing, efficient operations, timely billing of service fees to clients, and submission of billing status reports. Orders pharmaceutical drugs & supplies from the lowest cost vendors and originates/maintains low cost vendor purchase agreement contracts. 8. Personnel: Personnel Administration & staff evaluations, including daily supervision of various personnel working under the direction of the Pharmacist. Reporting: Responsible for preparation and submission of monthly Board reports for the Pharmacy department's objectives and performances to the Administration. Clinical Intervention Program Qualitative participation in clinical intervention program through consistent issue identification, provision of reliable best practice/literature-based recommendations, follow-through with prescribers and other clinicians. Quantitative work, measured by the completion and documentation of interventions consistent with the average established for each pharmacy department/shift. Communicates with nurses and physicians in a timely manner about therapeutic issues or problems. Follows up on formulary compliance issues and/or communicates issues to management. Documents interventions, patient contact, and multidisciplinary communication consistent with Sharp policies and procedures. When applicable, participates in scheduled rounds in their care area. Oversees Pharmacy Resident and/or PharmD student participation on rounds. Works collaboratively with the interdisciplinary team to maximize individual patient outcomes. Pharmacy Operations Medications are prepared and dispensed accurately and in a timely manner in accordance with recognized standards of practice, legal and regulatory requirements, and hospital pharmacy policies and procedures. Assures controlled substances are dispensed with accuracy. Maintains integrity of all record keeping and assists with narcotic inventories as required per policy and procedures. Demonstrates proficiency with and utilization of automation and advances in technology (EMR System, Automated Dispensing Cabinet system (Pyxis), IV Workflow Management system (DoseEdge), physician order entry, etc.) Demonstrates knowledge of age specific medication delivery systems (e.g. NICU admixtures, NICU and pediatric oral syringes, adult TPN / TNA solutions, final admixture concentration etc.) Demonstrates knowledge and competency of IV Admixture / Aseptic Technique including the standards of USP 797, 800 and 795. As applicable, demonstrates knowledge and competency of chemotherapy preparation. Within entities that provide medical emergency response, participates in Code-Blue emergencies as required, mixes and labels medications as needed, and provides information. As applicable, understands investigational drug protocols and procedures for investigational drug research studies, implementation, administration, and record keeping. Participates in cost management initiatives including: managing appropriate Inventory levels to decrease staleness & wastage and keep Inventories low; reduce waste attempts to reduce expired meds and use short dated items before they expire, etc. Borrow loan paperwork complete. Oversees pharmacy operations and provides direction to Pharmacist Residents, Pharmacist Interns, Pharmacy Technicians, and other non-licensed personnel. Safe Medication Practice, Regulatory, and Quality Improvement Knowledge of and active participation in concurrent medication safety event (MSE) monitoring programs, as demonstrated by interventions and RL submissions. Qualitative work demonstrates thoughtful and thorough identification, evaluation and analysis of medication safety events. Performs initial analysis of MSE's and assists in the research and identification causative factors of events. Performs non-voluntary monitoring (i.e. trigger reports, harm monitors, alerting orders). Participates in ongoing data collection for medication use evaluations (MUE) as assigned. Completes monthly assigned nursing unit / drug storage area inspections on time. Completes paperwork including inspection log, documents discrepancies, and makes recommendations for improvement. Brings critical issues to the attention of management. Ensures security of medication storage and compliance with Medication Management (MM) standards (i.e. all meds are labeled, patient own meds are checked, bedside medications have associated physician orders, no expired meds, and the physical security of medications is maintained). Assure compliance with hospital, integrated health system and department policies and procedures. Monitor compliance with state and federal laws and regulations related to the practice of pharmacy as well as standards and regulations for other regulatory agencies. Participate in assigned department, hospital, and health-system committees or projects.
