The typical pay range for this role is:
$60.00 - $73.00This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.

Serves as a clinical resource for the provision of pharmaceutical care and clinical pharmacy services for the entire medical center. Builds effective relationships with other healthcare professionals and departments within the medical center.
Develop, manage and mentor antimicrobial management program (AMP)
Responsible for managing and monitoring the drug formulary: completing drug class reviews, minimizing non-formulary procurements, suggesting appropriate, cost effective therapeutic alternatives to medical staff and implementing automatic therapeutic substitutions when appropriate.
Maintain professional competency sufficient to meet current and future needs of the facility.
Identify, design, analyze and report drug utilization reviews to promote rational and safe drug therapies and determines action plan based on results.

Maintain appropriate controlled substance records.
Train and participate with Disaster Team drills and real-event responses as assigned. Participate in quality and regulatory reviews and audits.
Work with the Pharmacy Director to identify and develop processes to meet or exceed all regulatory standards. Prioritize work and lead others to comply with facility medication turn-around time policy and procedure.
Act as manager in charge as assigned. Supervises and assist pharmacy technicians and other support staff effectively. Effectively and proactively intervene in conflicts between pharmacy and medical staff regarding drug therapy issue resolutions.
Work with the director to establish pharmacy goals and objectives of clinical programs and responsible for implementation and monitoring within budgetary targets.

Participate with various nursing and medical staff committees as necessary. Coordinates all activities related to the Pharmacy and Therapeutics Committee.
Responsible to coordinate the pharmacy review of medication orders in physician order sets, protocols or other evidenced based medication use guidelines
Responsible for routine publication of the pharmacy newsletter and/or other forms of written and electronic communication
In conjunction with Pharmacy Director and the Pharmacy Management team, responsible for the development and review of pharmacy policies and procedures. Work with the Pharmacy Director, Operation Manager and Medication Safety Officer to identify and develop processes to meet or exceed all controlled substance diversion prevention and monitoring standards.
Work with Pharmacy Operation Manager for the oversight of staff scheduling to ensure appropriate staff coverage for clinical programs. Share pharmacy operation responsibility with Pharmacy Operation Manager. In conjunction with pharmacy director and operations manager, responsible for staff performance evaluations. Qualifications:
Active Pharmacist license in good standing with the Florida Board of Pharmacy. ACLS certification is preferred.
A Doctor of Pharmacy (Pharm. D.) from an ACPE-accredited college of pharmacy is required.
Minimum of 1 year as a Pharmacy Clinical Coordinator or one of the following: Atleast 5 years of hospital pharmacy experience required; preferably within area of focus, completion of hospital PGY2/ pharmacy specialty residency or fellowship, Specialty BCPS certification, other certification (e.g. FCCP, FASHP).

The Medication Adherence Pharmacist is an innovative and patient-focused clinical pharmacist embedded within a defined market of primary care clinics. This role operates under a collaborative practice agreement to proactively manage medication adherence, chronic disease state control, and quality performance metrics. The pharmacist will partner directly with physicians, advanced practice providers, nurses, and care teams to optimize medication use, improve patient outcomes, and enhance CMS Star Ratings and HEDIS quality measures. This position requires advanced clinical judgment, effective patient engagement strategies, and the ability to implement evidence-based interventions that address adherence barriers, reduce hospitalizations, and improve overall care quality and scores.
**ESSENTIAL JOB DUTIES/RESPONSIBILITIES:**
+ Conduct targeted outreach to patients identified as at risk for non-adherence to chronic medications, focusing on CMS quality measure drug classes (e.g., diabetes, hypertension, cholesterol) and other high-priority therapeutic areas.
+ Review and verify prescriptions and medication order for appropriateness, including safety and efficacy.
+ Operate under a collaborative practice agreement to adjust medication therapy, order and interpret lab results, and manage chronic disease regimens to support adherence and clinical goals.
+ Perform comprehensive medication reviews, addressing cost, access, side effects, and complexity barriers to adherence
+ Collaborate with providers to implement care plans, streamline prescription workflows, and ensure timely refills.
+ Track and report adherence-related KPIs, including Proportion of Days Covered (PDC), polypharmacy measures, and specialty adherence metrics.
+ Provide education to patients, caregivers, and clinic staff on the importance of medication adherence
+ Represent the pharmacy department on relevant committees, payor calls,
+ quality teams, and market strategy groups within the market
+ Assist the department in addressing physician consultations as needed. Provide timely retrieval of reliable drug information from available sources.
+ Provide verbal and written consultations concerning medication therapy on a daily basis.
+ Assisting the Director to provide effective and efficient use of all resources and for ensuring budget and value proposition targets are met.
+ Performs other duties as assigned and modified at manager's discretion.
KNOWLEDGE, SKILL AND ABILITIES:
+ Knowledge of and experience with state and federal insurance programs
+ Familiarity with appropriate local, state, and federal pharmacy-related regulations
+ Ability to work effectively, both independently and as part of a team
+ Ability to supervise and communicate effectively with staff, management and executives
+ Strong organization and time management skills and multi-tasking abilities
+ Must be detail oriented
+ Ability and willingness to travel locally, regionally and/or nationally up to 10% of the time; flexible to work evening, weekends and/or holidays as needed
+ Spoken and written fluency in English
EDUCATION AND EXPERIENCE CRITERIA:
+ Must have a License that is in good standing with Board of Pharmacy in the state of Florida.
+ Doctorate of Pharmacy is required (PharmD); Pharmacy residency training is preferred, but not required
+ Collaborative Practice Experience or Ambulatory Care Experience preferred, but not required
+ Experience with supervising and/or supporting Pharmacy Technicians
+ Experience in a managed care setting (HMO, MSO, PBM) is a plus
+ Certified to provide immunization is a plus
+ Completion of Medication Therapy Management certificate program or Ambulatory Care Certificate a plus
