Responsibilities:
This position involves providing patients with comprehensive specialty pharmacy services. The Clinical Pharmacist responsibilities include providing clinical services to patients seen at the University of Illinois Hospital & Health Sciences System. The selected individual will focus on specialty medications management to ensure and promote the safe, appropriate, efficacious, and cost-effective use of drug therapy with the goal of achieving positive outcomes that improve the patient's health status and quality of life. Other responsibilities include supporting drug distribution processes, policy development, and population-based care improvements.
Patient care management includes ensuring patients receive specialty medications through UI Health ACHC and URAC accredited pharmacy. This involves developing and documenting specialty pharmacy care plans, initiating treatment, and providing patient education and training. Additional duties include performing and documenting clinical interventions such as adverse event management and adherence support, evaluating treatment outcomes, maintaining communication with clinic providers, and accurately documenting all activities in the electronic medical record. Clinical Pharmacist duties also include working closely with pharmacy technicians, students, medical staff and patients in providing patient-centered clinical pharmacy services. The academic responsibilities of the clinical faculty include the provision of didactic lectures and precepting entry level Doctor of Pharmacy students, PGY1 residents and PGY2 residents. Scholarly activities, including presentations and publications, are an expectation as well.
Minimum Qualifications
* Candidates must be a graduate of an ACPE accredited College of Pharmacy PharmD program.
* Must be eligible for licensure as a Pharmacist by the State of Illinois upon employment and duly licensed within earliest time-frame allowable by The State of Illinois Department of Professional Regulation.
* Candidates must have completed an ASHP accredited PGY1 residency and/or have equivalent clinical experience.
