Ensure the pharmacy complies each month with the perpetual inventory requirement of Schedule II drugs found in Regulation 18VAC110-20-240.
Notify the Board of any theft or unusual losses of drugs as soon as discovered.
Ensure all security measures are in compliance and operational.

POSITION SUMMARY:
Clearway Health is looking for a Clinical Pharmacist to support the Pediatric Hematology/Oncology Specialty Pharmacy space, providing expert patient-centered pharmaceutical care to pediatric patients with hematologic and oncologic conditions. This role will support the safe, effective, and timely use of highrisk and high-cost specialty medications, including chemotherapy, immunotherapies, biologics, oral Oncolytics, and supportive care agents. This position plays a critical role in medication safety, therapy optimization, patient and caregiver education, medication access, adherence monitoring, accreditation, and specialty pharmacy clinical program growth.
ESSENTIAL RESPONSIBILITIES/DUTIES:
Provide comprehensive medication therapy management for pediatric hematology and oncology patients.
Review, verify, and monitor hem/onc pharmacotherapy for appropriateness, dosing accuracy, efficacy, and safety, including weight and body surface area-based dosing.
Monitor for adverse effects, toxicities, drug- drug interactions, and therapy complications.
Support oral oncolytic management, adherence monitoring, and toxicity assessment.
Perform medication assessments and reassessments prior to medication dispense.
Collaborate with pediatric hematologists, oncologists, advanced practice providers, nurses and other members of the care team.
Assist pharmacy liaison with PA submission, appeal letters, insurance troubleshooting, etc.
Participate in clinics, case conferences, tumor boards, and multidisciplinary rounds as appropriate.
Provide evidence-based pharmacotherapy recommendations aligned with pediatric oncology protocols and guidelines.
Support specialty pharmacy dispensing workflows for hem/onc therapies, including limited distribution drugs and REMS programs.
Facilitate medication access by assisting with prior authorizations, appeals, manufacturer programs, and patient assistance resources.
Coordinate with payers, manufacturers, and specialty distributors to prevent delays in therapy initiation or continuation.
EDUCATION:
A PharmD or bachelor's degree from an accredited College of Pharmacy is required.
Completion of PGY-1 Pharmacy Residency or 3+ years working as a clinical pharmacist in direct patient care is required.
Completion of a PGY-2 in oncology/hematology, transplant, or pediatrics is preferred.

Qualifications

* Dispenses controlled substances and maintains perpetual inventory of controlled substances assuring legal requirements and departmental policies and procedures are consistently followed.
* Provides functional supervision for support personnel. Monitors attendance and work quality organizes and prioritizes resources based on workload and productivity and directs the workflow of support personnel. Ensures departmental standards and time frames are met.
* Enters and verifies computer entries and data for drug orders. * In decentralized setting may instruct patients/families on the safe use of medications and counsel them on potential adverse effects.
* Monitors drug therapy of assigned patients and takes appropriate action to ensure optimum patient drug therapy drug selection and minimal adverse reaction. Assures drugs/dosages are accurate based on the therapeutic intent and the patient's physiologic functions. Reviews patient profile to determine possible contraindications with the prescription.
* Participates autonomously in-patient care rounds. Reviews patient specific data to evaluate patient's status and medication needs and develops a therapeutic plan. Counsels/educates patient on their medications e.g. to correct dosing schedule and duplicate therapy.
* Responds to evaluative and/or non-evaluative drug information requests.
* Assures compliance of prescribers with clinical practice guidelines and other drug policies or third-party prescription benefit management procedures minimizes non-formulary drug use eliminates therapeutic duplication. Makes recommendations for appropriate alternative medications duplicate therapy and dosage changes.
Minimal Qualifications
Education
* Doctoral degree or an equivalent Pharmacy degree from an accredited college of Pharmacy required
Experience
* PGY-1 or 3 years of direct patient care experience in a hospital setting required
Licenses and Certifications
* PHARMD - Licensed Pharmacist in the state of practice required
* Board certification required within 1 year of eligibility
Knowledge Skills and Abilities
* Verbal and written communication skills. Basic computer skills preferred.
