The Pharmacist Level 1 plays a key role in supporting high-quality patient care within a hospital setting. This is an inpatient-only position; experience limited to mailing prescriptions from a hospital system will not be considered.
Key Responsibilities
Accurately and efficiently prepare and dispense medications in accordance with hospital policies and procedures.
Provide pharmacokinetic and age-appropriate dosing recommendations based on current clinical literature.
Support clinical teams by offering pharmacological guidance and answering questions related to formulary and non-formulary medications.

Additional responsibilities include evaluating cancer-related medications such as antineoplastic and supportive therapies, contributing to research and quality initiatives, and partnering with revenue cycle teams to expand billable services and strengthen compliance and reimbursement.
Education & Licensure Requirements:
Doctor of Pharmacy (PharmD) degree required
Active license to practice pharmacy in the State of New York
Board Certification in Pharmacotherapy or equivalent certification preferred

Experience:
Completion of a PGY2 Residency in Oncology is strongly preferred. In lieu of a PGY2, completion of a PGY1 Pharmacy Residency and a minimum of 3 years of direct clinical experience in oncology pharmacy practice is required.
Demonstrated expertise in managing chemotherapy, immunotherapy, and targeted therapies for adult oncology patients
Experience applying geriatric oncology principles, including frailty assessment, dose adjustments, polypharmacy management, and supportive care
Familiarity with Adolescent and Young Adult (AYA) oncology, including psychosocial, reproductive, and survivorship considerations
Experience in designing, conducting, and managing clinical research

Overview

Process prescriptions accurately or supervise others in the process in accordance with current laws, ethical standards and Company policies.
Provide outstanding customer service and patient counseling in the areas of prescription and OTC medications, and other evolving health care needs.

Duties and Responsibilities:
Serve as the or Supervising Pharmacist (SP) as required. Responsible for ensuring regulatory compliance with Federal and state pharmacy laws and regulations.
Participate in the financial performance of the department including, but not limited to, sales growth, maximizing gross margins and productivity, controlling inventory, payroll and expenses.

The Pharmacist will oversee and provide direction related to quality, clinical and compliance for Apotheco Pharmacy operations initiatives in compliance with state and federal laws. The Pharmacist will report to the Pharmacist-in-Charge. Must have excellent customer service skills and be detailed oriented.
Work with Pharmacist-in-Charge to maintain and increase efficiency, quality, and customer service levels in the daily operation, as well as execute on department goals. Continue performing essential pharmacist functions of a staff pharmacist.

10. Proficient at pharmacy software system and electronic ordering of all medications.
11. Perform final check on prescriptions annotating on the prescription and scanning in changes
12. Call physicians when applicable for refills, medication changes, and prior authorizations.
13. Making sure all drugs have a minimum of 6 months prior to expiration
14. Perform return to stock duties for prescriptions not picked up within 14 days
15. Counsel patients on all new prescriptions

Expired drugs: responsible for determining procedures along with manager of fulfillment pertaining to disposal and credits of expired drugs. IX. Inventory: aid in the process of inventory validation as needed. X. Variances with inventory validate along with manager of inventory and finance any variances. XI. C2- all C2 prescriptions must be counted by a technician and then recounted by a pharmacist. C2 count one section per week or delegate to another pharmacist and record in controlled substance log. XII. Other activities as determined by management.

Overview
Blythedale Children's Hospital is seeking a Clinical Pharmacist. In this role, you will provide comprehensive pharmaceutical care to pediatric patients through the development, implementation, coordination, and maintenance of clinical pharmacy services that promote desirable patient outcomes through safe and effective drug therapies.
Perform all Staff Pharmacist duties as needed.
Schedule: This position will have variable shifts consisting of 7.5 hours per day throughout the week.
Schedules vary from 7am-3pm, 8am-4pm, 9am-5pm, 11am-7pm, 1pm-9pm.
Schedule includes rotational weekend coverage as well as rotational holiday coverage.
Schedule will also include clinical coverage and staff coverage.
On-call at nights rotated with all pharmacists when pharmacy is closed weekdays/weekends/holidays.
This position includes a $2,500.00 differential with the base hourly salary.
Responsibilities
Attend and hold clinical pharmacy meetings such as Medication Safety Committee, P&T, and Antibiotic Stewardship Committee meetings.
Participate in medical rounds, IDCP, VAC team central line rounds, and various other meetings. Contribute to the improvement of pharmacy metrics.
Play a critical role in enhancing the education of healthcare and non-healthcare team members on treatments related to pediatric patients.
Conduct medication usage evaluations and research projects in collaboration with other healthcare professionals to identify areas of medication use optimization.
Participate in site-based and multidisciplinary committees as needed.
Qualifications
Minimum 1-year hospital pharmacy experience or completion of Hospital Pharmacy residency program.
Education, Licensure, and Certification
PharmD degree from an accredited School of Pharmacy.
Current New York State Pharmacy License or license eligible by New York State.
Preferred:
Completion of PGY1 Pharmacy Residency.
Completion of PGY2 Pharmacy Residency.
