Basic Qualifications

What you'll do in the role:
Provide exceptional patient care and service to health center providers.
Actively provide clinical pharmacy services included in the Patient Management Program.

Reports on Quality Improvement Projects throughout the pharmacy locations.
Review and recommend medication alternatives, medication consolidation, and dose modification consistent with updated clinical guidelines.
Actively collaborative with Engagement Specialists to ensure ongoing medication access for all patients.
Review payor formularies and complete prior authorizations and appeals as necessary for patients.
Obtain and document all required information in specialty pharmacy software as required to maintain specialty accreditation.
Participate as a member of the Quality Management Committee, when needed.
Participate in URAC and ACHC specialty pharmacy accreditation preparation and visits, as required.
Precept pharmacy interns and students.
Any other duties as assigned.
Education & Licensure Requirements:
PharmD, BS in Pharmacy, or equivalent FPGEC (NABP).
Unrestricted pharmacist license in good standing in the state of Indiana.
Must be willing to obtain and maintain licenses in multiple states.
Other Skills or Qualifications:
3+ years of direct patient care experience in pharmacy is preferred.
BLS/CPR are required (or must be willing to obtain within 1 month after start).
Immunization delivery certification is preferred.
2+ years of specialty pharmacy experience is preferred.
PGY1 residence or equivalent work experience preferred.
CSP credential is preferred.
Excellent written and oral communication skills.

Responsibilities
Applies expert knowledge of drugs and related protocols, to verify physician orders for medications using pharmacy information system. Checks drug and dose in relation to stated patient issue and addresses automated alerts from the system regarding drug type, dose, and other metrics.
Supervises Pharmacy Technicians to ensure the compliant and efficient operations of the pharmacy. Checks all work of Pharmacy Technicians in line with State Boards of Pharmacy regulations and provides guidance on complex prescriptions, pharmacy automation systems, orders, and other issues.

Description:
This position exists to provide comprehensive, value based, medication-related care to achieve definite outcomes that improve a patient's quality of life. The incumbent acts in collaboration with healthcare professionals in the shared responsibility for patient drug therapy outcomes. Key job duties may consist of ensuring safety and effectiveness of the medication use process, processing and dispensing medications per order or prescription, medication therapy monitoring, regulatory compliance, maintaining oversight over pharmacist extenders, maintaining the security of medications, and protecting the confidentiality of patient information. Additionally, is competent at providing drug information support for medical staff, ensuring continuity of care, and identifying potential challenges in common situations.
Requires Doctor of Pharmacy (PharmD) or Bachelor's of Science in Pharmacy prior to 2006.
Successful completion of an ASHP accredited PGY1 or PGY2 residency may be required.
Requires 0-3 years of relevant experience.
Requires current IndianaPharmacist license or eligibility for Indiana licensure as a Pharmacist.
Board certification or other specialized training may be required.
Basic Life Support certification through the American Heart Association (AHA) may be required.

Job Summary
Working in a hospital system pharmacy outsourced to Cardinal Health, the Pharmacist II adjusts and dispenses medication to patients and drives the compliant and efficient operations of the pharmacy. The Pharmacist verifies physician orders using the pharmacy information system, makes adjustments according to protocols for a given medication, and provides patient education. This job also supervises Pharmacy Technicians and manages compliance tasks related to accreditation, narcotics control, and inventory management.
Cardinal Health manages the pharmacy at AMG Specialty Hospital - Hancock Campus, and we are currently recruiting for a Part-Time Hospital Pharmacist to work 24 hours a week with shared on call responsibilities. Hours of the facility are Monday-Friday 8AM-5PM and 8AM-12PM weekends.
Responsibilities
* Applies expert knowledge of drugs and related protocols, to verify physician orders for medications using pharmacy information system. Checks drug and dose in relation to stated patient issue and addresses automated alerts from the system regarding drug type, dose, and other metrics.
* Supervises Pharmacy Technicians to ensure the compliant and efficient operations of the pharmacy. Checks all work of Pharmacy Technicians in line with State Boards of Pharmacy regulations and provides guidance on complex prescriptions, pharmacy automation systems, orders, and other issues.
* Adjusts dosages based on established protocols for each drug, ensuring that dosages are appropriate for the patient's condition and in light of other medications the patient is using and completes clinical tasks related to antimicrobial stewardship.
* Educates patients on the proper use and expected effects of medications, especially when patients are using multiple medications, are beginning new regiments, or are being discharged.
* Manages narcotics and inventory control and compliance, including regular documentation and completion of checklists. Audits outdated medication in states where Pharmacy Technicians are not permitted to do so.
* Contacts Nurses and Physicians as necessary to verify orders and obtain additional information regarding the order.
* Assists the Pharmacy Director in maintaining the pharmacy's accreditation and in implementing operational changes within the pharmacy, such as the use of new systems or adjustments of standard procedures.
Qualifications
* Licensed, "good standing" pharmacistin the state of Indiana or willing to become licensed in the state
* In-patient hospital pharmacy experience preferred
* Flexibility to work various shifts
* IV experience preferred or willing to obtain
* Ability to hear, write, and speak clearly in order to communicate with customers and health care professionals
* Strong customer service skills.
