What Health System Pharmacy contributes to Cardinal Health
Health System Pharmacy is responsible for providing customized pharmacy program solutions that reduce costs and improve patient care quality for hospitals, health systems and other integrated healthcare providers may also consult with and advise healthcare team on prescribed medications, supplies and related processes.
Responsibilities
* Responsible for the day-to-day pharmaceutical care and management of patients.
* Interacts with patients, health care providers, clinics, and prescribers involved with the patient.
* Review prescriptions prior to dispensing and verify all questionable prescriptions.
* Ensure appropriate storage of products in the facility and compliance with all laws, regulations, and accrediting agencies (e.g. NABP) standards.
* Maintain the required number of continuing education credits to maintain credentials as required by each state license.
* Assist with shipping logistics on orders before and after they ship.
* Other duties as assigned.
Qualifications
* Bachelor's degree in pharmacy required, PharmD preferred.
* Must be licensed to practice pharmacy in Texas.
* 3-5 years related pharmacy experience preferred.
* Past retail or specialty pharmacy experience preferred.
* Must be available to work ON SITE between the hours of 8:00am-7:30pm CST Monday - Friday and on-call semi-annually.

ProSidian seeks a Registered Pharmacist to support engagement with The Veteran's Affairs Consolidated Mail Outpatient Pharmacy (CMOP). This position serves as a key member of the facility leadership team providing services that include, but are not limited to, pharmaceutical verification of prescription orders of legend, over the counter (OTC), medical-surgical (Med/Surg), and controlled substances to patients requiring care. In addition, the candidate shall provide patient consultation, quality improvement, and supply control duties.
Furthermore, the candidate shall direct the delivery of pharmacy services, efficient and effective drug distribution, and education to improve patient quality of life, minimize therapeutic risk, and enhance clinical outcomes. Education:

Apply knowledge of state and federal laws and regulations pertaining to the legal operation of a pharmacy to comply with laws, regulations, and policies governing pharmacy operations;

What You'll Do
The Clinical Pharmacist will play a critical role in executing Suvida's Healthcare model to strengthen the provider-patient relationship and engagement by enhancing the quality and affordability of care. This individual will focus on supporting providers and patients through medication management support, pharmacist-led clinical programs, and practice transformation. You will be responsible for optimizing medication therapy to improve clinical outcomes such as blood pressure and A1c control, reduce preventable ED and hospital utilization, and monitor adherence to medication regimens and care plans. The clinical pharmacist meets the patient where their needs are (virtually, in-office, and at-home) to identify, resolve, and prevent medication-related problems. As time allows, the pharmacist will also participate in various committees and initiatives (e.g. program development for chronic diseases (i.e. COPD, DM, CHF), HEDIS strategies to close gaps, clinic operations, etc.).
