Position Summary:
The Antibiotic Stewardship Clinical Pharmacist will serve as a unit-based pharmacist, verifying orders and providing specialized clinical services in antibiotic stewardship and infectious disease. This role will be a critical drug information resource for nurses, physicians, patients, and families.
Responsibilities:
Provide direct patient-centered care and integrated pharmacy services in collaboration with physicians, nurses, and other healthcare professionals.
Routine attendance on medical rounds (for decentralized shifts) with consistent documentation of pharmacist interventions.
Deliver medication therapy management with a focus on antibiotic stewardship and infectious disease protocols.
Ensure pharmaceutical care programs are appropriately integrated throughout the institution.
Assist in development and implementation of facility-specific protocols, guidelines, and reference documents related to antimicrobial therapy management.
Conduct Medication Use Evaluations (MUEs) with a focus on antibiotics/ID, and recommend necessary changes to pharmacotherapy practice.
Serve as a resource in antibiotic stewardship initiatives, ensuring best practices and compliance with institutional standards.

Experience

Strongly Preferred: Infectious Disease training and/or experience in antibiotic stewardship
Certifications

Preferred: Board Certification in Pharmacotherapy (BCPS) or actively working toward certification

About the Role:
The Director of Pharmacy Benefit Manager Oversight operating within the Health Care Access Bureau is authorized, pursuant to M.G.L. c. 176Y, to oversee the practices of licensed Pharmacy Benefit Managers, including their operations with insured health plans that either provide or arrange for the delivery of health care services through networks of providers, or that employ utilization review systems to evaluate the medical necessity and appropriateness of requested health care services. The primary purpose of the Director of Pharmacy Benefit Manager Oversight is to assist the Commissioner of Insurance and Deputy Commissioner of the Health Care Access Bureau, in the coordination of pharmacy benefit reviews, which comprises but is not limited to the following duties:
Oversee the review of Pharmacy Benefit Managers applications for licensure. Review is of carriers' operations, including utilization review, quality assurance, contracts with pharmacies and prescription drug manufacturers, and of their disclosure practices in relation to regulatory standards and statutory requirements.
Manage the review of pharmacy benefit manager consumer communications, including explanations of benefits given to consumers, employers or health care providers.
Collaborate with staff from the Health Policy Commission to oversee the collection of information related to Pharmacy Benefit Managers and the processing of insured health plans' pharmacy benefit denials and appeals as overseen by OPP.

Job Summary
The MGH Department of Pharmacy is seeking a Transitions of Care/Boarders Clinical Pharmacist to join our team at the Main Campus. This position will be focused on collaborating with multidisciplinary team members to optimize care for Boarder patients (primarily medicine patients) residing in the Emergency Department and supervise pharmacy technicians/learners participating in the daily conduct of medication histories. This is an exciting opportunity for those looking to lead, build, and expand future services to include all aspects of care transitions for patients, and optimize care for Boarder patients at our Main Campus. Pharmacists in this role are positioned to fulfill their professional aspirations and make a positive impact on the lives and care for our patients. This position is for an 8-on 6-off rotating schedule. Emergency medicine experience is not required.
Clinical Pharmacists assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Clinical Pharmacists proficiently provide direct patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel to deliver optimal medication therapy to patients with a broad range of disease states. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. Pharmaceutical care services include, but are not limited to, assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacists serve as a departmental resources and liaisons to other departments, hospital personnel, or external groups. They proficiently manage the appropriate use of medications according to professional and regulatory standards. They also conduct practice-related research and patient care, quality, and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. Finally, Clinical Pharmacists provide medication and practice-related education/training and actively serve as preceptors for pharmacy residents and Doctor of Pharmacy students (MGH Pharmacy Residency website: **********************************************************

Pharmacist [State License] - Generic - HR Only required Massachusetts Controlled Substances Registration [MCSR - Massachusetts] - Massachusetts Department of Public Health preferred
Experience
Experience working as a pharmacist 2-3 years preferred
Knowledge, Skills and Abilities
- Extensive knowledge and ability in all areas of the department including: IV Admixtures, Drug Information, Ambulatory, Manufacturing, and Drug Distribution.
- Attention to detail.
- Demonstrate appropriate knowledge of principles of growth and development over the life span of the neonate, adolescent, adult, and geriatric patient Possess ability to assess patient data relative to age specific needs Provide care as described in the department's policies/procedures.
- Works relatively independently.
- Participates in department quality improvement efforts.
- Participates in the training of medical, nursing and allied health professionals.
- Must complete 20 continuing education credits per year to maintain license.
- Familiarity with pharmacy practice settings, legal requirements and limitations, prescription information and calculations, drug identification and generic equivalents, drug manufacturing, packaging and labeling information, and proper drug handling and storage practices.
