Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies and appropriateness of drug and dose. Assists with pharmacokinetics consult service and renal drug dosing per hospital protocol. Reads, extracts, and interprets information in patient medical records accurately. Detects and reports suspected adverse drug reactions accurately and in a timely manner. The pharmacist is responsible to manage patients' drug therapy to ensure optimal, cost effective care delivery and improve health outcomes. This role supports the full physician panel, regardless of health plan and may involve multiple clinic locations. The expectation is that associates will work from home 3-4 days a week and in the clinic 1-2 days a week. Candidates MUST be licensed & located in: San Antonio, Texas. Role Specific Responsibilities (executed in person and virtual) Comprehensive oversight of drug therapy for high-risk patients Execute clinical protocols as allowed per organization and state regulatory agencies Optimization of drug coverage benefits for appropriate care and reduced costs (preferred formulary products, lower cost alternatives and home delivery pharmacy) Participation in interdisciplinary team meetings for patient care plan development Patient and provider consultation in the management of chronic disease states Disease state education for patients on appropriate use of medications and self-management Physician and staff education Post-discharge medication reconciliation for high-risk transition of care patients Document all activity within clinic EMR and other required pharmacy or clinical management systems Support CMS Stars quality measures for successful clinic performance Vaccine administration if needed Use your skills to make an impact Minimum Requirements Active and unrestricted pharmacist license within the state(s) of the provider practice locations Pharm.D. or other advanced pharmacy degree 2+ years' experience as a practicing pharmacist Certified in basic life support (BLS) and immunization administration Excellent organization and communication (verbal and written) skills Ability to work in a team environment and support common goals Strong attention to detail; able to concentrate without distraction in a fast-paced environment Ability to prioritize multiple tasks, anticipate and solve problems Intermediate level or higher competency with MS Office programs such as Word, Excel and Power Point Preferred Requirements Clinical residency in ambulatory care or similar discipline Board certification in geriatric pharmacy 2+ years working in an ambulatory care/primary practice setting Familiarity with electronic medical record (EMR) systems Experience in managing drug therapy for patients with chronic conditions Identify and implement innovative solutions for improved efficiencies and outcomes Multilingual (preferably English/Spanish) Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $115,200 - $158,400 per year This job is eligible for a bonus incentive plan.
