The Pharmacist plays a vital role in ensuring the accuracy and quality of prescriptions within the Pharmacy setting. This position is responsible for providing Pharmacy Services within the Inpatient and Outpatient Retail Pharmacy Department and the Nemours Children's Hospital in Orlando FL. The Clinical Staff Pharmacist will review, clarify, verify, and provide recommendations for medication orders and be responsible for the dispensing of medications, and the activities surrounding dispensing, throughout the hospital and its associated clinics at Medical City. Position Responsibilities * Reviews interpret and verify medications orders for accuracy using sound principles, accounting for the pharmacology of the drug, pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism, and elimination with responsibility for establishing and overseeing the delivery and preparation of unit dose and parenteral pharmaceuticals. * Carries out rules and regulations pertaining to the professional policies of the pharmacy department and ensures compliance with State and Federal regulations concerning Controlled Substances. * Provide pediatric pharmaceutical services to improve drug usage and therapeutic outcomes including advising physicians and practitioners on issues concerning drug therapy, the inherent toxicity of drugs, drug-drug, drug-food interactions and side effects, as well as assisting in the prescribing of appropriate doses. * Responsible to provide accurate, adequate, and timely drug information to the professional staff and to patients and their families if needed. * Coordinates the activities of pharmacy technicians engaged in the preparation, labeling and delivery of medications in accordance with federal, state, and local laws and regulations pertaining to the dispensing of drugs, controlled drugs, investigational drugs, and the maintenance of required records. * Reads, extract and interprets information in patient's charts accurately. * Participates in the quality improvement and medication use review activities of the department. Collects data; conducts quality monitors and inspections; maintains logs, records, and other documentation as assigned. * Promote the use of the Nemours Children's Hospital drug formulary by converting non-formulary orders to formulary when possible, but facilitate procurement of non-formulary drugs when necessary. * Establish and maintain an open communication process throughout the hospital. * Works with other NCH colleagues to engage the patient and their family in medication discussions, planning and educational sessions to support Patient and Family Centered Care. Job Requirements * Bachelor's degree required, Pharm D preferred. * Minimum of six (6) months experience required. Prior work experience to include months/years working as a Pharmacy Technician, a Pharmacist Intern and/or while in a Residency program. Job Summary Cardinal Health manages the pharmacy at PAM Health Rehabilitation Hospital of Ocoee, we are currently recruiting for a PRN Pharmacist to work as needed during facility hours and on-call. The facility is open Monday through Friday 8:00am to 4:30pm and on-call after hours, weekends and holidays. Must be able to cover at least one week of on-call per month. Our Pharmacists provide pharmaceutical care services to the patients of the hospital including, but not limited to oversight, monitoring, preparation, dispensing and proper documentation of patient medication therapy as well as providing drug information, patient information and oversight of information and dispensing systems. The successful candidate will work with the team to ensure compliance with Federal, State and local laws and regulations as well as proper storage and documentation of pharmaceuticals. Responsibilities Interprets medical orders (verbal and written) and transcribes to computerized patient medication profiles accurately. Maintains accurate, complete patient medication profiles. Compounds and dispenses pharmaceuticals, including sterile, chemotherapy, and parenteral nutrition products accurately. Issues controlled substances to patient care areas and maintains records as required by law. Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose. Qualifications Bachelor's degree in Pharmacy or PharmD required. Licensed, “good standing” pharmacist in the state of Florida required. In-patient/hospital pharmacy experience preferred.
