CHA's work extends far beyond patient care. It has a robust Department of Community Health Improvement and operates the nationally accredited Cambridge Public Health Department. It collaborates closely with local governments and non-profits to improve health and reduce barriers to care. The CHA Pharmacy Enterprise is a dynamic department with cutting edge clinical services and state of the art automation and technology. It includes three inpatient pharmacies, inpatient and ambulatory clinical pharmacy services, four retail / specialty pharmacies, a PACE pharmacy, and a PGY-1 pharmacy residency program. We precept APPE and IPPE students from 5 area colleges of pharmacy. We are an ACPE-accredited provider of continuing pharmacy education. Forty-five of our clinical pharmacists from all areas of operation are credentialed members of the CHA Medical Staff and execute a number of drug therapy management protocols through collaborative practice agreements with supervising physicians. All of the services we provide are performed with a focus on optimizing patient outcomes and safety, increasing patient satisfaction, and contributing to CHA's financial sustainability. The Clinical Pharmacist Specialist in Inpatient Psychiatry is responsible for providing pharmaceutical care for our adult and geriatric psychiatry units. Primary, essential job functions include daily rounding with a multidisciplinary care team, ensuring rational drug therapy, serving as a clinical role model for staff, precepting PGY1 pharmacy residents and APPE students from area colleges of pharmacy, providing education and training to healthcare professionals, contributing to the P&T Committee, and representing the Pharmacy Enterprise on related CHA-wide multidisciplinary committees. As a credentialed member of the CHA medical staff, the clinical pharmacist specialist works under a collaborative practice agreement with a supervising physician to execute defined drug therapy protocols. A commitment to patient safety, customer satisfaction, efficient/effective use of resources, teamwork, innovation, and performance improvement is required. Responsibilities: • Regularly provides clinical pharmacotherapy services via inpatient care team rounding. • Monitors drug therapy using implicit and explicit criteria to ensure safety and effectiveness. • Performs formal and informal medication use consults with appropriate documentation. • Provides direct, effective, and individualized education and counseling to patients and/or family members/caregivers. • Demonstrates and documents value to healthcare team through pharmaceutical care contributions/interventions. • Promotes medication safety among colleagues and on behalf of patients in specialty area. • Assesses need, develops and/or refines clinical pharmacy programs. Assesses clinical and financial impact of efforts on an ongoing basis. • Evaluates medication use (i.e., formal usage evaluations); effects measurable improvements in medication use process. • Assists in maintenance of Drug Formulary. Reviews new requests, non-formulary requests, and evolving literature on behalf of P and T Committee. • Develops drug use restrictions, guidelines, and protocols for consideration by P&T Committee. • Leads staff development efforts in clinical area(s) • Serves as a preceptor for PGY1 pharmacy residents and APPE students. MINIMUM QUALIFICATIONS: Graduate of an accredited college/university with B.S. in Pharmacy or Pharm.D. degree is required. Board Certification is required. (or agreement to obtain certification within 2 years of employment) Certification by a relevant specialty body/discipline is desirable. Completion of an ASHP-accredited residency is desirable. Licensure: Must have a current license in good standing from the Massachusetts Board of Registration in Pharmacy or be eligible for licensure in MA. In lieu of an active MA license, candidates must attain MA licensure prior to working 1000 hours at the Cambridge Health Alliance (CHA). Failure to attain MA licensure within this time period will result in the discontinuation of employment at CHA. The clinical pharmacist provides comprehensive medication management services that integrate clinical services, order management, patient/staff education and research. This position is responsible to an immediate supervisor/manager.Position Summary:The Advanced Therapeutics clinical pharmacist provides patient centered clinical pharmacy services, including but not limited to comprehensive medication reviews, adherence monitoring interventions, and targeted medication reviews, that seek to optimize clinical, economic, and patient outcomes. The Advanced Therapeutics clinical pharmacist designs and implements targeted medication and disease management programs. Advanced Therapeutics clinical pharmacists are responsible for improving the patient's experience, safety, and achieving positive clinical and financial outcomes. This position is responsible to an immediate supervisor/manager.Major Responsibilities: The following key performance areas (Clinical, Operations/Distribution, Financial, Safety and Regulatory, Research and Education) are performed by the clinical pharmacist: Clinical Performance: Provides medication management services in a collaborative manner that consist of the following: Review patient medication history, educate patients on drug therapy, establish desired outcomes, develop/modify therapeutic plans, determine alternatives for achieving outcomes, develop drug monitoring plans, monitor patient outcomes, document medication related issues in electronic health record, interpret laboratory data, review patient medication profiles, monitor pharmacokinetics of drug therapy, monitoring for potential adverse drug - drug, drug-lab, drug-disease state, and drug - food interactions. Maintains and updates a patient profile with demographics (e.g. height and weight), diagnosis, allergies, and current medications. Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interaction. Provides drug information to health care professionals and to patients. Documents clinical interventions and activities in a timely manner. Documentation may include progress notes in the EHR, annual comprehensive medication reviews with written summaries in the CMS's standardized format, and patient specific documents, i.e., medication-related actions plans and personal medication records. Counsels patients on discharge medications as appropriate. Performs clinical review and verification of medication orders. Performs medication order entry when needed. Discusses medication order clarifications with the prescriber, documents interventions, and communicates medication order changes as needed. Facilitates medication discharge planning and smooth transition to alternate sites of care and access to an ambulatory pharmacy. Maintains accurate and proper patient medication profiles, monitoring to ensure the proper choice of medication, dosage form, strength, length of therapy and interaction potential of the prescribed medication. May work under Collaborative Practice and protocols as authorized by providers. Supervise work of pharmacy technicians involved with MTM and transitions of care. Financial Performance Promotes the use of formulary drugs by converting non-formulary orders to formulary drugs when possible according to P&T protocol. Coordinates procurement of non-formulary drugs when necessary. Conducts targeted drug program and drug usage evaluations as needed and reports results to Department Managers/ Pharmacy and Therapeutics Committee. Identifies and acts upon cost avoidance in drug therapy by promoting cost effective regimens, drugs, and by reducing waste. Facilitates cost saving programs such as IV to PO and therapeutic substitution. Manages medication inventory, expiring products, and recalls. Safety and Regulatory Performance Detects, monitors, documents, and reports adverse drug reactions and medication errors. Utilizes drug use reports to identify adverse drug events. Participates in Joint Commission and other accreditation and regulatory activities such as National Patient Safety Goals and Department of Pharmacy Quality Improvement Plan. Research and Education Performance Precepts Intermediate Pharmacy Practice Experience (IPPE) rotation students, and Advanced Pharmacy Practice Experience (APPE) rotation students, and pharmacy residents consistent with academic credentials and clinical experience. Participates in continuing education programs, meetings, training programs and related activities. Prepares educational materials and participates in information sharing activities for the health care community and general public. Standard Staffing Level Responsibilities Complies with established departmental policies and procedures. Attends variety of meetings, conferences, seminars as required or directed to meet departmental goals. Demonstrates use of Quality Improvement in daily operations. Position Qualifications:License/Certification/Education: Required: Doctor of Pharmacy (Pharm.D) degree; OR Bachelor of Science in Pharmacy (BSPharm) degree with 3 years hospital experience. Licensure with the Massachusetts Board of Registration in Pharmacy. Preferred: ASHP Accredited Residency in Pharmacy Practice (PGY1) is recommended. Pharm.D. degree with minimum of 1 year of hospital experience.
