Monitor for adverse effects, toxicities, drug- drug interactions, and therapy complications. Support oral oncolytic management, adherence monitoring, and toxicity assessment. Perform medication assessments and reassessments prior to medication dispense. Collaborate with pediatric hematologists, oncologists, advanced practice providers, nurses and other members of the care team. Assist pharmacy liaison with PA submission, appeal letters, insurance troubleshooting, etc. Participate in clinics, case conferences, tumor boards, and multidisciplinary rounds as appropriate. EDUCATION: A PharmD or bachelor's degree from an accredited College of Pharmacy is required.
