POSITION SUMMARY Responsible for ensuring the proper compounding, dispensation, review, and verification of prescribed medications within regulatory guidelines, company policies and procedures. Provide pharmacy consulting services to patients regarding the effective usage of medications, awareness with drug interactions and offering preventive healthcare services. Support the efficient workflow of the pharmacy and assist the Pharmacy Director in identifying ways to optimize pharmacy financials, inventory management and enhance patient experience. ESSENTIAL FUNCTIONS Assist Pharmacy Director with establishing procedures that promote the efficient workflow of the pharmacy including assigning roles, coordinating activities and soliciting employee suggestions. RESPONSIBILITIES: Evaluate patients responses to medications and help plan appropriate drug therapies based on physician diagnoses. Order and review lab tests to monitor the effectiveness and safety of medications. Collaborate with medical staff to start, adjust, or stop medications for chronic conditions (e.g., hypertension, diabetes, asthma). Manage medications that require regular monitoring and dosage adjustments (e.g., antithrombotics, phenytoin). Administer prescription and over-the-counter medications per established protocols. Assess nutritional needs and prescribe therapeutic nutrition (enteral/parenteral) for hospitalized patients. Review medical and medication histories to identify and resolve drug-related issues. Work closely with other healthcare professionals to coordinate patient care. Perform comprehensive pharmacy services, including reviewing prescriptions for accuracy and safety. Ensure proper dosing, check for drug interactions, and verify allergy information. Oversee the preparation and dispensing of medications by pharmacy technicians. Provide training to clinical staff on new medications, best practices, and treatment comparisons. Consult with specialists to optimize pharmaceutical care. Document significant drug interactions and communications with prescribers. Report adverse drug reactions in line with accreditation standards. Ensures the safe and effective use of medications throughout the Hospital in compliance with State and Federal regulations and consistent with current best practices in evidence-based medicine. Acts as a medication information resource to patients, medical staff, nursing staff, and ancillary department personnel. Provides high quality comprehensive pharmaceutical care appropriate to the age of the patient which contributes to positive patient outcomes. Accurately review and verify medication orders, resolving medication therapy interactions, allergies, and incompatibilities. Compound medications using standard formulas and processes such as weighing, measuring, and mixing ingredients. Participated on hospital committees such as the Pharmacy and Therapeutics Committee, Antimicrobial Stewardship Committee, Department Director Meeting, Infection Control Meeting, and Quality Committee. Prepares department budget for personnel, operating expenses, and capital equipment. Responsible for fiscal operation of the department. Responsible for the implementation, monitoring and evaluation of quality improvement initiatives. Maintains formularies, information resources, and references appropriate for the scope of services. Prepare IV admixtures and extemporaneous parenteral products, demonstrating proper technique. Ensure accuracy and appropriateness of medication distributed throughout the hospital system. Provide concise, applicable, comprehensive, and timely responses to requests for medication information. Reports medication errors and adverse drug events, and effectively contributes to the timely resolution of current event and prevention of future events. Provide leadership and oversight for Pharmacy Technicians and Interns. Provide and document patient medication education per protocol. Monitoring and controlling inventory to ensure medication availability, quality of products available and minimizing waste. Qualifications Current registered pharmacist in the state of New Mexico Bachelor's or Doctorate in Pharmacy.

BPS (Board of Pharmaceutical Specialties in Oncology) certification or Pharmacist Clinician certification preferred. Continuous education to maintain licensure is required. NM Sterile Product Preparation Certification is required. Safe Handling of Hazardous Medications course (or similar course approved by pharmacy director/manager) is required within 2 months. SKILLS: Effective verbal and written communication skills Advanced mathematical skills required Keyboarding skills are preferred (must be able to develop competence using hospital pharmacy software). Word processing skills are preferred Must have the ability to work with medical staff, nursing staff and other healthcare employees Knowledge of USP 797 and USP 800 regulations is required. EXPERIENCE: Three years experience as a hospital pharmacist is preferred.
