Role: Provides direct face-to-face care within progressive clinical pharmacy practice; Responsible for managing chronic disease patient panel with focus on managed care, high-risk patients with uncontrolled chronic disease states (Diabetes, HTN, HF, HLP, Obesity [cardiometabolic]); Optimizes medication therapy through collaborative scope of practice.

MAJOR RESPONSIBILITIES
Demonstrates expertise in clinical practice and grows and manages their own panel of patients with chronic disease using the standards of comprehensive medication management services, including assessment of all medications, development of individualized care plans to ensure appropriate drug therapy and prevention/resolution of drug related problems, and follow-up evaluation. Provides clinical support to others for more complex cases, applying in-depth knowledge in specialty area. Solves complex problems and acts as a resource to assist others. Pharmacists involved in the provision of services covered under a collaborative practice agreement will be required to demonstrate competence and achieve credentialing in the areas of chronic disease they are managing.
Leads/participates in medication safety advancements and performance improvement activities for pharmacy services in specialty service line. Routinely participates in specialty work groups including councils, committees, and ad hoc teams.
Serves as clinical pharmacy resource to site or system service line to ensure clinically appropriate cost-effective medication therapies. Works with closely and collaboratively with other patient care practitioners (physicians, APCs, nurses, etc.) to deliver optimal drug therapy to patients and facilitates change implementation within the service line.
Actively supports clinical outcomes including improved quality measures, decreased hospital admissions, chronic disease management clinical indicators, patient satisfaction, etc.
Leads training and educational topic discussions regarding ambulatory care pharmacology and therapeutics, within site or service line, for pharmacy team members, medical staff, students, and residents. Routinely serves as preceptor for interns, students, and/or residents. Provides input to development and improvements in training programs.
Demonstrates a commitment to continued individual professional growth and leadership in the advancement of the pharmacy profession. Contributes to literature through publications, research and/or presentations.
MINIMUM EDUCATION AND EXPERIENCE REQUIRED
License/Registration/Certification Required: Pharmacist license issues in the state(s) of practice. Specialty certification required to be obtained within one year of hire.
Education Required: : Bachelor's Degree in Pharmacy or PharmD. PGY1 ASHP residency required.
Experience Required: Typically requires 5 years of experience or relevant specialized PGY2 residency.
