Qualifications/Skills:
• Current BLS certification through the American Heart Association (AHA) required

ProSidian seeks a Registered Pharmacist to support engagement with The Veteran's Affairs Consolidated Mail Outpatient Pharmacy (CMOP). This position serves as a key member of the facility leadership team providing services that include, but are not limited to, pharmaceutical verification of prescription orders of legend, over the counter (OTC), medical-surgical (Med/Surg), and controlled substances to patients requiring care. In addition, the candidate shall provide patient consultation, quality improvement, and supply control duties.
Furthermore, the candidate shall direct the delivery of pharmacy services, efficient and effective drug distribution, and education to improve patient quality of life, minimize therapeutic risk, and enhance clinical outcomes. The candidate's leadership attributes in Pharmacy management shall ensure the delivery and attainment of patient care quality metrics, evaluate the performance of Pharmacy Technicians duties, and drive client satisfaction outcomes.
This is a Contract Contingent position and the Registered Pharmacist services shall be performed at the: Veteran's Affairs Consolidated Mail Outpatient Pharmacy (CMOP) facility at: 3725 Rivers Ave Suite 2, North Charleston, SC 29405. The selected candidate is expected to fulfil the duties to a satisfactory level of professionalism and quality.

Qualifications
Education:
Bachelor of Science in Pharmacy degree that is recognized by the Accreditation Council for Pharmacy Education (ACPE) or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.
Licensure:
A current, active, valid, and unrestricted license to practice pharmacy in the State of South Carolina or in possession of an active license from another state that enjoys reciprocity with the state of South Carolina.
The candidate must have passed the North American Pharmacist Licensure Examination (NAPLEX) and the Multistate Pharmacy Jurisprudence Examination (MPJE).
Experience:
Demonstrable proficiency in the compounding of prescriptions, drugs, chemicals, and pharmaceutical supplies required in the operation of a pharmacy;
Dispense medications prescribed by physicians and other health practitioners and providing information to health practitioners and patients about proper usage of medications and side effects;
Conduct drug utilization reviews, counsel patients on the proper use of medication while evaluating medication use patterns and outcomes for patients leveraging adherence best practices;
Apply knowledge of state and federal laws and regulations pertaining to the legal operation of a pharmacy to comply with laws, regulations, and policies governing pharmacy operations;
Provide oversight of CMOP facility by developing and implementing operational procedures, maintain accurate records and document actions taken while safeguarding the confidentiality of medical records;
Apply inventory control expertise in establishing medication-handling procedures for the storage and preservation of medications;
Communicate effectively, both orally and in writing, with fellow medical professionals and patients; develop harmonious professional relationships with clinic staff; and
Supervise Pharmacy Technicians in the performance of work-related to sterile products, prescription filling, and ward/clinic supply.

This position requires regular communication with prescribers, office staff, patients/caregivers, and a variety of other healthcare and industry professionals. This position must also meet departmental staff development requirements. The pharmacist may participate in departmental or health system committees and the provision of education. The amount of time allocated to operations and clinical services depends upon staffing and overall pharmacy workload.
Essential Functions

Provides accurate medication order-entry, drug distribution, drug administration, diagnostic testing and patient care. Confirms the appropriateness of drug therapy as it relates to the care of specific, specialized patient populations including neonates, pediatrics, adolescents, adults, and geriatrics. Seeks to minimize turn-around-time for products and services.
Performs all pharmacist duties specific to pharmacy including:
a. Patient Education
b. Patient Care Management
c. Reviews and evaluates prescriptions for safety, appropriateness, and clinical efficacy
d. Completes prior authorizations/appeals for medications using EMR data and/or direct contact with providers
e. Provides initial and ongoing education on medications including associated supportive care
f. Creates and monitors individual care plans for patients
g. Develops/maintains relationships with medical providers and office staff personnel
h. Refers patients to provider, other healthcare professionals, or community resources as needed
i. Participates in pharmacist "on call" program where applicable.
Provides concise, applicable, timely responses to requests for drug information.
Oversees pharmacy support personnel; checks and verifies the accuracy of all tasks performed. May also function as lead or charge pharmacist within the department to monitor workload and workflow.
Completes lab reviews, pharmacy consults and provides clinical services, with appropriate documentation, via the department's initiatives.
Acts as liaison between pharmacy and assigned patient care area solving distribution problems and monitoring drug supplies for proper storage, etc.
Provides experiential education and training for pharmacy technicians, students and new team members.
Represents the department in its collaborative effort to integrate pharmaceutical care throughout the patient's continuum of care.
Performs other duties as assigned.

This is a non-management job that will report to a supervisor, manager, director, or executive.
Minimum Requirements
Education - Bachelor's degree from an ACPE-approved college of Pharmacy. PharmD preferred.

In lieu of an active Pharmacist license, may be a graduate of an ACPE accredited US college of Pharmacy with an pharmacist intern certificate pending initial licensure or have completed requirements for reciprocity pending board of pharmacy interview for licensure.

Basic Qualifications
BS in Pharmacy or Pharmacist Degree from an accredited educational institution.
Current pharmacist licensure in the states within the district.

Job Summary

This job also supervises Pharmacy Technicians and manages compliance tasks related to accreditation, narcotics control, and inventory management.
Responsibilities
Applies expert knowledge of drugs and related protocols, to verify physician orders for medications using pharmacy information system. Checks drug and dose in relation to stated patient issue and addresses automated alerts from the system regarding drug type, dose, and other metrics.
Supervises Pharmacy Technicians to ensure the compliant and efficient operations of the pharmacy. Checks all work of Pharmacy Technicians in line with State Boards of Pharmacy regulations and provides guidance on complex prescriptions, pharmacy automation systems, orders, and other issues.
Adjusts dosages based on established protocols for each drug, ensuring that dosages are appropriate for the patient's condition and in light of other medications the patient is using and completes clinical tasks related to antimicrobial stewardship.
Educates patients on the proper use and expected effects of medications, especially when patients are using multiple medications, are beginning new regiments, or are being discharged.
Manages narcotics and inventory control and compliance, including regular documentation and completion of checklists. Audits outdated medication in states where Pharmacy Technicians are not permitted to do so.
Contacts Nurses and Physicians as necessary to verify orders and obtain additional information regarding the order.
Assists the Pharmacy Director in maintaining the pharmacy's accreditation and in implementing operational changes within the pharmacy, such as the use of new systems or adjustments of standard procedures.
Other duties as assigned.
Qualifications
Bachelors degree in pharmacy or PharmD.
Licensed, “good standing” pharmacist in the state of South Carolina.
In-patient hospital pharmacy experience preferred.
IV experience preferred.
Ability to hear, write, and speak clearly in order to communicate with customers and health care professionals.
Strong customer service skills.
May require vendor credentialing.
