About Pharmacy Medication History In Med History, our goal is to obtain the best possible medication history for each patient admitted to the hospital. This step is crucial to completion of Joint Commission accreditation-mandated medication reconciliation. Medication History Technicians claim patients from a work list based on the patient's location and the tech's scheduled shift assignment. Technicians follow a structured interview with the patient or caregiver and document the information into the electronic medical record. Discrepancies are communicated directly to the provider/pharmacist for resolution. Scope of Work Provides technical assistance in the documentation and clarification of medication history lists under the direction of the pharmacist. Proficient in stocking shelves as evidenced by alack of errors. Participates in inventory management practices, including appropriate stock pars, and yearly physical inventory. Conducts appropriate patient charging and crediting, applies interdepartmental billing processes, and charge capture review. Generates required reports, reviews billing reports, and initiates corrections to discrepancies. Files reports as required.
