Facility: Dayton Children's - Main Campus Department: Clinical Pharmacy Schedule: Full time Hours: 40 Job Details: The Medication History Pharmacy Technician, with supervision of a pharmacist, assists in the process of obtaining medication history for patients who will be admitted to the hospital. The focus of this technician is the accurate collection of prescription information, allergy information, patient and/or family/guardian interview, and documentation of those activities. The complete collection of medication information may also include contacting pharmacies, long-term care facilities, and prescribers.
Additionally, the Medication History Pharmacy Technician works under the supervision of the pharmacist following established policy and procedures set forth by the hospital and pharmacy department. This role meets the requirements of being a qualified pharmacy technician as established by the State Board of Pharmacy and holds a Certified Pharmacy Technician license from a State Board of Pharmacy accredited agency. The technician adheres to safety practices to ensure quality care for patients and maintains competencies outlined in pharmacy department policies. The technician shows familiarity with pharmacy practice settings, legal requirements and limitations, prescription information and calculations, drug identification and generic equivalents, drug manufacturing, packaging and labeling information, and proper drug handling and storage practices.

Position Overview

* You will serve as an Assigned Trainer and competency evaluator for new hire and ongoing sterile compounding and support staff in USP 797 and 800 required core skills, knowledge, and required competencies such as Garbing and Hand Hygiene, Aseptic Technique, and cleanroom suite cleaning assessments.
* Compound sterile medications.
* Assist with compounding equipment calibration and ensure appropriate use of equipment during daily sterile compounding operations.
* Perform the formulation of the solutions and aseptic filling of the solutions
* Perform daily, weekly and monthly cleanroom suite support responsibilities including, but not limited to, compounding materials and supplies preparation, equipment calibration, cleaning, dishwashing, and sterilization or depyrogenation of equipment, according to facility SOP's.
* Document compounding-related and daily, weekly, and monthly activities according to facility SOPs and as required by USP 797 and 800.
* Coordinate with the Pharmacist-in-charge (PIC), Sterile Compounding Manager, Pharmacist, and/or Lead Sterile Compounding Pharmacist for personnel, quality, compliance and regulatory matters.
* Communicate with Lead Sterile Compounding Pharmacists or Sterile Compounding Pharmacists to report on issues in the cleanroom suite.
* Utilizes equipment as instructed by the pharmacist.
* Completes training as required by USP 797 and 800 (and related chapters).
You Have:
* 2+ years sterile compounding experience or 1+ year sterile compounding plus 1+ year nonsterile compounding experience.
* Current registration as a Certified Pharmacy Technician in the State of Ohio and National Certification (CPhT)
* Certified Compounded Sterile Preparation Technician (CSPT) or willingness to complete in the first year of employment.
* Strong communication skills and can work independently.
* Full time role 40 hours per week.
Preferred:
* 3+ year sterile compounding experience in nonsterile to sterile compounding or 1+ year in sterile-to-sterile compounding.
* Prior experience at a sterile pharmaceutical manufacturer.
* 1+ year experience in leadership role.
* 1+ year as a sterile or nonsterile trainer.

* This position will require working with Hazardous Drugs (HD) and would require that Personal Protective Equipment (PPE) be worn for the length of working with these drugs. These items would include gloves, respiratory protection, gown and other items as required.
