Position Summary -

Job Description
This project is in the proposal stage, we're awaiting to be awarded the contract.
International Health and Medical Services is looking for an experienced Pharmacist to manage the procurement, dispensing, and monitoring of medications to ensure safe and effective pharmaceutical care for patients. This role includes collaborating with healthcare providers to optimize medication management, provide drug information, and contribute to the overall health and well-being of the patient population.
Key Responsibilities
Responsible for the day-to-day operation of the medication delivery system and assures that the integrity of all medications is maintained and is properly stored.
Responsible for participating in quality assurance activities regarding medications to maintain quality patient care and cost-effective medication management.
Responsible for the records and reports related to the purchase, receipt and disposition of drugs and related supplies.
Responsible for the delivery of pharmaceutical care including patient centered, outcomes-orientated pharmacy practice.
Work in concert with the patient and other healthcare providers to promote health; prevent disease; and assess, monitor, initiate and modify medication use to ensure that drug therapy regimens are safe and effective.
Dispense formulary medications.
Prescriptions are filled at local pharmacies which are part of the Script Care Network (or other designated Pharmacy Benefits Manager).

Qualifications
Basic Requirements/Certifications:
Current and unrestricted license to practice pharmacy in the state of Oregon
American Heart Association (AHA) Basic Life Support (BLS).
Trained in Basic First Aid.

Position Summary
We are seeking a clinical pharmacist with a focus on industry relations. The domain expertise requires both foundational clinical pharmacy experience alongside an interest and passion for partnering closely with pharmaceutical and biotechnology companies, and other vendor partners who engage in contracting opportunities such as rebate management, and clinical programs. Verify the accuracy and appropriateness of medications prepared and dispensed from the pharmacy.
Assists in the reconciliation of medications, education of patients regarding their home medication regimens, and coordination of the discharge medication plan with the interdisciplinary team. Documents activities in the patient medical record.
Monitors therapeutic response and adverse effects of medications to guide drug therapy. Consults with provider when questions or problems are identified and documents actives in the patient medical record. Appropriately reports all adverse drug reactions, drug defects, and medication errors.
Responds accurately to requests for drug information from prescriber's, nurses, patients and patient's families; independently reviews and interprets the literature and applies this information to specific patient care situations.
Assists in the development of treatment guidelines to be used in patients with specific diagnosis. Documents activities in the patient medical record, pharmacy monitoring profile, drug use evaluation data collection or adverse drug reaction reporting program.

Required Qualifications
A current Pharmacist's license issued by the Oregon State Board of Pharmacy.
Inpatient hospital pharmacy practice: Ability to meet the pharmaceutical care needs of the following patient care types: Acute care, psychiatric, rehabilitation, skilled nursing, long-term care, obstetric, oncology, chemically dependent, HIV, home care, hospice.
BS in Pharmacy or Pharm.D. PLUS Practice Residency/Equivalent Experience or Specialist Certification
(BCPS, BCOPS, etc)

Specialist Certifications (BCPS, BCOPS, etc) State of Oregon Pharmacy Preceptor licensure

The pharmacist will experience competing demands from nurses, physicians, pharmacy staff and other healthcare professionals, as they attempt to influence clinical decision-making, clinical policies and practices and work flow. The pharmacist must deal with intense situations on a daily basis. In addition, within the healthcare setting there can be significant lack of control over the work pace, with frequent interruptions (work is often dictated by external factors) that may lead to mental fatigue or stress.
