Job Summary
The Massachusetts General Hospital is seeking highly motivated pharmacists to join our expanding team of Primary Care Pharmacists. This position offers a rare and exciting opportunity to contribute to the strategic development and implementation of new clinical pharmacy services within a primary care setting! We are hiring one full-time pharmacist (1.0 FTE), with flexible options available for candidates who may be interested in a part-time role (0.8 FTE).
In this role, clinicians will collaborate with interdisciplinary and specialty services to manage chronic disease states through a Collaborative Drug Therapy Management (CDTM) agreement. Initial efforts will focus on the management of hypertension, diabetes, and dyslipidemia, expanding to additional disease states in the future. Key responsibilities for the clinician include conducting comprehensive patient visits, creating and implementing patient centered, guideline directed disease state management strategies, and providing ongoing monitoring. Recognized as an integral member of the team, pharmacists will collaborate with physicians, advanced practice providers, nurses, population health coordinators, medication access coordinators, and community health workers to support holistic, patient-centered care and improve health outcomes. Non-direct patient care responsibilities include participation in quality improvement initiatives, research, and the training and education of multidisciplinary team members and learners.
Qualifications
Clinical Pharmacists assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy. Clinical Pharmacists proficiently provide direct patient-centered care and integrated pharmacy operational services with physicians, nurses, and other hospital personnel to deliver optimal medication therapy to patients with a broad range of disease states. These clinicians are aligned with target interdisciplinary programs and specialty services to deliver medication therapy management and to ensure pharmaceutical care programs are appropriately integrated throughout the institution. Pharmaceutical care services include, but are not limited to, assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, adjusting care according to patient response, and providing clinical interventions to detect, mitigate, and prevent medication adverse events. Clinical Pharmacists serve as departmental resources and liaisons to other departments, hospital personnel, or external groups. They proficiently manage the appropriate use of medications according to professional and regulatory standards. They also conduct practice-related research and patient care, quality, and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice. REQUIRED:
Must be eligible for licensure to practice pharmacy in the Commonwealth of Massachusetts
Postgraduate residency/fellowship training is preferred
Board Certification (BCACP, BCPS) by examination, or equivalent certification, in the relevant specialty is preferred, required within 18 months of hiring
Must meet all Board of Pharmacy and MGH requirements to enter into a collaborative drug therapy management agreement.
Massachusetts Controlled Substance Registration license, National Provider Index number, and a Drug Enforcement Administration Registration number, when applicable

Safety and Regulatory Performance
Detects, monitors, documents, and reports adverse drug reactions and medication errors.
Utilizes drug use reports to identify adverse drug events.
Participates in Joint Commission and other accreditation and regulatory activities such as National Patient Safety Goals and Department of Pharmacy Quality Improvement Plan.
Research and Education Performance
Precepts Intermediate Pharmacy Practice Experience (IPPE) rotation students, and Advanced Pharmacy Practice Experience (APPE) rotation students, and pharmacy residents consistent with academic credentials and clinical experience.
Participates in continuing education programs, meetings, training programs and related activities.
Prepares educational materials and participates in information sharing activities for the health care community and general public.

Job Summary
Under direction of the Senior Operations Manager, PET Production and Cyclotron, the nuclear pharmacist provides a variety of services, ensuring compliance in the procurement, preparation, compounding, manufacturing, quality control testing, dispensing, distribution, and monitoring of radiopharmaceuticals. Duties and Responsibilities for Nuclear Pharmacists in the MGH PET Nuclear Pharmacy include but are not limited to:
1. Assumes overall responsibility for quality oversight of the FDA regulated, cGMP PET manufacturing facility and PET nuclear pharmacy for the manufacturing, compounding, quality control testing, dispensing and packaging of PET radiopharmaceuticals.
2. Serves in a quality assurance (QA) role to ensure compliance with all departmental procedures, FDA guidance and regulatory filings.
3. Reviews and releases batches of PET radiopharmaceuticals manufactured under 21 CFR 212, cGMP PET drug manufacturing regulations.
4. Ensures compliance with all state, federal, and local regulations concerning these activities.
5. Maintains inventory and ensures suitability of all raw materials and components used in the production of PET radiopharmaceuticals
6. Performs set up, operation, and documentation of computer controlled automated synthetic devices dedicated to the production of PET radiopharmaceuticals.

Performs complex analytical testing on manufactured and/or compounded radiopharmaceutical including, thin layer chromatography (TLC), high performance liquid chromatography (HPLC), gas chromatography (GC), radionuclide identity and purity testing with high purity germanium solid state detectors, sterility and pyrogen testing.
8. Works with PET radiochemistry staff to troubleshoot radiopharmaceutical production and optimize yields.
9. Works with cyclotron facility staff to ensure quality and availability of PET radionuclides for manufacturing and compounding of PET radiopharmaceuticals.
10. Maintains detailed records of operation and validation of all processes taking place in the nuclear pharmacy and PET production facility.
11. Ensures that all shipments of radioactive material meet all regulatory requirements.
12. Works with MGH radiation safety staff to ensure that all tasks are performed in the PET nuclear pharmacy are in compliance with regulatory and prudent radiation safety principles.

Key Responsibilities:
* Controlled substances distribution, non-sterile compounding oversight, inventory functions oversight, discharge prescriptions, and hospital patient daily medication supply provision review.
* Interpreting and evaluating medication orders to ensure drug and dosage are appropriate for patient and to identify possible adverse reactions, contacting physician as needed.

Minimum Qualifications
Education:

Relationships:

QUALIFICATIONS:
REQUIRED:
* Must be eligible for licensure to practice pharmacy in the Commonwealth of Massachusetts
* Postgraduate residency/fellowship training
* Board Certification by examination, or equivalent certification, in the relevant specialty is preferred, required within 18 months of hiring
* Must meet all Board of Pharmacy requirements to enter into a collaborative drug therapy management agreement, when applicable
* Massachusetts Controlled Substance Registration license, National Provider Index number, and a Drug Enforcement Administration Registration number, when applicable
PREFERRED:
* Completion of ASHP-accredited PGY1 and PGY2 Solid Organ Transplant Pharmacy Residency or Fellowship

+ Appropriately dispenses pharmaceuticals, including sterile products, chemotherapy, biologics, controlled substances, and investigational medications

A minimum of 3 years of experience in hospital pharmacy, ambulatory care, or sterile compounding, or 5 years of retail pharmacy experience with 2 years of oncology experience, is required. Alternatively, completion of a PGY1 residency meets the experience requirement. The position also requires a license to practice pharmacy in the Commonwealth of Massachusetts, with additional licensure needed for some satellite locations.
