Basic Qualifications
BS in Pharmacy or Pharmacist Degree from an accredited educational institution.
Current pharmacist licensure in the states within the district.

Under the general supervision of the Administrative Director for Primary Care and Associate Chief Clinical Officer, and reporting to the CHMG Patient Refill Center Pharmacist Lead, the CHMG Patient Refill Center Staff Pharmacist is responsible for providing clinical support for centralized prescription renewal and prior authorization services while also managing prescription renewal requests and prior authorizations submissions per protocol. Education
PharmD degree or Bachelor's degree in Pharmacy from an accredited program.
Certification, Registration & Licensure

Board Certification is desirable (BCPS or BCACP).
Experience
A Pharmacy residency is desirable.
Ambulatory care or community pharmacy experience is desirable.
Experience working within a structured health care organization is desirable.
Responsibilities
* Manages prescription renewal requests and prior authorization submissions per protocol.
* Support clinical workflows and processes for prescription renewal and prior authorization services, including optimizing drug therapy based upon insurance coverage, patient adherence, and available therapeutic options.
* Coordinates prescription related education and training for CHMG Patient Refill Center staff.
* Identifies and resolves actual or potential medication related problems.
* Takes ownership of prescription renewal issues and follows them to completion.
* Documents, audits, and monitors prescription renewal interventions and their outcomes.
* Support continuous improvement of policies, procedures, and protocols related to prescription processes.

What You'll Do

Recommends appropriate medication doses and adjustments as indicated by patient conditions. Respond to critical patient situations (ie: Trauma, codes) to provide pharmacy support as part of the medical response team.

Requirements
Education - PharmD or BS in Pharmacy required. Completion of pharmacy residency preferred.
