Main Responsibilities
Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions

Minimum Qualifications
Bachelor's Degree in Pharmacy is required

Accurately maintains calendar for laboratory tests, compounding, delivery, and documentation updates
Obtains and processes patient clinical information to allow for processing refills and supplies at least 24 hours in advance of patient's need.
Plans work to ensure cost-effective delivery methods are utilized.
Oversees the work of pharmacy support personnel and ensures their job duties are completed in a timely manner.
Delegates non-clinical tasks to assigned support personnel.
Prepares for and actively participates in patient care conferences including Nutrition Support Rounds.
Participates in the on-call process and documents on call activities in the patient medical record, if appropriate, and communicates with relevant personnel.
Assists in the training and supervision of pharmacy support personnel, pharmacy interns and pharmacy-profession students.
May assume role in of the Pharmacist in Charge or general branch manager during temporary absences and/or vacancies as appropriate.

EDUCATION AND/OR EXPERIENCE:

Currently licensed as a registered pharmacist in state of employment.
A minimum of two years recent experience in an acute-care setting or one year recent experience in home infusion therapy
Experience and strong knowledge base in intravenous admixture services and intravenous therapy.
Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses and other healthcare professionals.
Must be able to work effectively with others to provide safe and effective drug therapy.
Must demonstrate ability to problem solve and supervise technical and other support staff.
Must be flexible with work schedule.
Good organizational and time-management skills.
Participation and membership in professional societies and organizations is desirable.

Responsibilities
