About the role:
As KabaFusion, we put an emphasis on patient care and positive clinical outcomes and, as a pharmacist with us, you are the front line. Whether that is working with providers, calling the patients themselves, or doing clinical review, you are an integral cog in the continuum of care the patient receives.
Essential Duties & Responsibilities:
Demonstrates ethical pharmacy practice and ensures compliance with all company policies and procedures, state and federal pharmacy rules and regulations, professional practice standards, and accrediting entities.
Assesses laboratory results and performs pharmacokinetic dosing.
Performs accurate prescription order entry to ensure medications are properly concentrated, without incompatibilities, stable and properly stored.
As a pharmacist, you bring:
Bachelor of Science in Pharmacy (Doctor of Pharmacy or a Master of Science degree in Pharmacy desirable) from an accredited college of pharmacy.
Current license as a registered pharmacist in state of employment.
1-2 years of recent experience and strong knowledge base in intravenous admixture services and intravenous therapy.

*JOB SPECIFIC COMPETENCIES* *Core Pharmaceutical Services* Core pharmaceutical services includes but is not limited to the following: a. Reviews, clarifies, and processes orders accurately and in a timely manner. b. Assures appropriateness of ordered medications by considering pertinent factors such as disease state, body surface area (BSA), age, height, weight, labs, and renal function.
c. Utilizes computer resources and clinical information to facilitate the provision of pharmaceutical care functions such as medication order entry, order verification, medication distribution, medication checking, and medication reconciliation.
d. Perform and document therapeutic interventions as appropriate.
e. Actively participate in event reporting using UHC Safety Intelligence.
*Pharmacy Area Supervision/ Responsibilities/Administrative/ Systems- Based Practice* Pharmacy Area Supervision/Responsibilities/Administrative/ Systems- Based Practice includes:
a. Provides supervision of Immediate Technical Staff working in the area including completion of job assignments, monitoring of lunch breaks, and assuring adherence to institutional and departmental policies and procedures (i.e., sterile and non-sterile compounding).
b. Ensure all tasks/assignments are completed in a timely manner to maintain appropriate workflow for optimal patient care.
c. Always maintain clean and organized work area.
d. Monitors area inventory and supplies, notifies appropriate personnel when shortages and/or outages noted.
e. Oversee the compounding of sterile and non-sterile admixtures.
f. Completes all assigned Quality Assurance and productivity reports including record keeping and appropriate follow up.
g. Researches/monitors problems, initiates corrective action and follow through until challenge is resolved.
c. Consistently provides input/suggestions for process improvements.
h. Completes/maintains all Institutional/Divisional requirements for licensure, mandatory education and testing. Including but not limited to EEE, Infection Control, age related competency, Code Blue, BLS Certification, TB testing, and IV process validation.
i. Develop, implement, and/or support special projects throughout the institution.
*Clinical/ Patient Care* Clinical/ Patient Care includes all of the following but not limited to:
a. Active participation in implementation and practice of identified clinical initiatives (e.g., discharge counseling, medication reconciliation, renal dose adjustments, etc.)
b. Conducts and documents patient education as appropriate.
c. Documentation of patient specific information in the electronic medical record.

We're looking for a Pharmacy Auditor, someone who's ready to grow with our company. In this position you will assist in overseeing the daily operations of the ADM, reports, audits and database management.

Job Duties & Responsibilities
Assists in the operations for the Pharmacy Automated Dispensing Machines (Omnicell).
Oversees adequate par levels of non-controlled substance medications are maintained in the ADMs as documented through Par vs. Usage reports and as evidenced by lack of inventory complaints from nursing and/or pharmacy areas.
Works with Pharmacy Computer Support and BioMed to follow up on technical or mechanical issues involving Omnicells.
Provides support for the inventory specialist during annual inventory (non-controlled substance medications).
Create new Omnicell configuration for non-controlled substances medications. Work with the Inventory Specialist (Controlled Substance/Omnicell) to coordinate the addition of new Omnicell.
Ensure Controlled Substance Compliance.
Documentation and Report.
Participates in the education of pharmacy students and new employees as documented by signature on the objectives and competencies form.
Informs manager in a timely manner regarding any compliant issues.
Performs other essential functions.

Pharmacist Major Responsibilities:
Evaluates physician medication orders for appropriateness of drug, dosage, potential interactions, and route of administration. Makes recommendations for therapy changes to the physician and facility staff as appropriate.

The Position:
Therapy Care Pharmacists utilize national and local resources to facilitate high quality care of a patients with specialty pharmacy needs. Coordinate activities with local or national pharmacy team members, including pharmacists, pharmacy technicians, nursing, intake/admissions, reimbursement, and sales staff to coordinate to ensure an efficient customer focused approach and optimal clinical patient outcomes
Documentation of all patient interaction and care coordination in the designated system of record for clear and consistent communication
Dispense specialty medication and supplies
