Basic Qualifications

Job Summary:
The Staff Pharmacist is responsible for the day-to-day operational duties associated with the dispensing of prescription medication as required to meet the needs of a long-term/post-acute care pharmacy. The Staff Pharmacist must adhere to the company's quality and customer service standards, policies and procedures, as well as the State and Federal guidelines.
Duties/Responsibilities:
Verification for completeness and accuracy of all new and refill labels and orders. Check for content and accuracy of all medication packaging and labeling including emergency drug kits and IV medications
Maintain resident database to include appropriate current and past medical history, appropriate clinical and laboratory data, and concurrent medication (including OTC and home remedies)
Assist in the proper ordering, handling, receiving, documentation, and storage of all controlled substances; assist in maintaining a perpetual inventory for schedule II drugs
Work with appropriate pharmacy technician in reconciling Controlled Substances perpetual inventory as required by management for compliance with state and federal regulations
Provide clinical expertise by responding to drug-related questions and concerns initiated by internal staff and external customers
Document all communication with physicians, nurses, other members of the healthcare team and patients/residents and their family members in the medical record
Preparation and/or supervision of all sterile IV drug products or solutions
Provide evening, weekend, and holiday coverage during rotation and as required by management
Follow all applicable government regulations including HIPAA
Other duties as assigned; Job duties may vary depending on business needs

We are looking for a Pharmacist to join our team of providing the highest quality of care, reliability, and support to our patients, caregivers, and families. In this role, the Pharmacist will be responsible for dispensing medications, providing clinical advice, as well as delivery services.
Shift: On call Sunday-Wednesday night and possible Saturday/ Sunday 12:00pm-4:00pm rotation
Responsibilities
Provides clinical consultations to patients as well as recommendations to nurses, physicians, and other hospice personnel
Evaluates medication profiles, compliance, and modifications to achieve desired therapeutic outcomes
Answers customer questions and resolves customer issues in a timely manner
Retrieves data in the pharmacy system
Manages medication orders
Ensures proper storage and security conditions for medications
Maintains a productive, orderly, and error-free work environment
Qualifications
Doctor of Pharmacy Degree or Bachelor of Science Pharmacy Degree
Current and good standing Pharmacist License
Strong communication skills, customer service, and problem-solving skills

Assist with medication preparation and dispensing, including insurance adjudication, counting, labeling, and point-of-sale transactions in accordance with pharmacy policies and regulations
Resolve insurance-related issues such as prior authorizations, step therapy requirements, and drug utilization reviews
Coordinate with providers, medical assistants, and clinical staff to clarify prescriptions and support continuity of care
Respond to patient, employee, and external inquiries in a professional and customer-focused manner
Manage inbound and outbound telephone communications as needed
Accurately handle cash register transactions and maintain proper documentation
Assist with inventory control, including ordering, receiving, stocking, and monitoring medications and supplies
Maintain cleanliness, organization, and safety of the dispensary workspace in compliance with regulatory and clinic standards
Provide coverage at other clinic pharmacy locations as needed

Work Environment & Physical Requirements
This role is performed in a clinical dispensary setting. The position requires standing and walking for extended periods, with intermittent sitting throughout the workday. The employee must be able to lift or move up to 10 pounds and perform tasks requiring manual dexterity, visual acuity, and verbal communication.
