What You Will Do

Essential Functions
* Verification and review of medication orders

* Age, sex, height, weight, and renal/hepatic function
* Pregnancy and lactation status
* Appropriateness of the dose, route, rate, timing and frequency of administration, indication, and therapeutic duplication of all medications
* Available laboratory values and microbiology results
Appropriateness of antimicrobial therapy regimens which may include anticipated/planned stopped dates.
Potential drug-drug, disease-drug, food-drug interactions, or contraindications.
Adherence with Sharp Pharmacy and Therapeutics Committee approved medication use guidelines/protocols.
Targeted or therapeutic drug monitoring and pharmacokinetics, and adjustment of drug regimens if necessary.
Review documented adverse drug reactions/allergies and report new occurrences.
Review for disease progression and resolution of symptoms (e.g., pain).
Communication of necessary information to pertinent health care providers, patients, caregivers, and encourage health promotion and prevention.
Assesses the patient's progression to achieving desired medication treatment goals.
At applicable entities, demonstrates knowledge and competency of chemotherapy dosing, monitoring, and established guidelines.
* Clinical Intervention Program
Qualitative participation in clinical intervention program through consistent issue identification, provision of reliable best practice/literature-based recommendations, follow-through with prescribers and other clinicians.
Quantitative work, measured by the completion and documentation of interventions consistent with the average established for each pharmacy department/shift.
Communicates with nurses and physicians in a timely manner about therapeutic issues or problems. Follows up on formulary compliance issues and/or communicates issues to management.
Documents interventions, patient contact, and multidisciplinary communication consistent with Sharp policies and procedures.

* Pharmacy Operations

Assures controlled substances are dispensed with accuracy. Maintains integrity of all record keeping and assists with narcotic inventories as required per policy and procedures.
Demonstrates proficiency with and utilization of automation and advances in technology (EMR System, Automated Dispensing Cabinet system (Pyxis), IV Workflow Management system (DoseEdge), physician order entry, etc.)
Demonstrates knowledge of age specific medication delivery systems (e.g. NICU admixtures, NICU and pediatric oral syringes, adult TPN / TNA solutions, final admixture concentration etc.)

As applicable, understands investigational drug protocols and procedures for investigational drug research studies, implementation, administration, and record keeping.
Participates in cost management initiatives including: managing appropriate Inventory levels to decrease staleness & wastage and keep Inventories low; reduce waste attempts to reduce expired meds and use short dated items before they expire, etc. Borrow loan paperwork complete.
Oversees pharmacy operations and provides direction to Pharmacist Residents, Pharmacist Interns, Pharmacy Technicians, and other non-licensed personnel.
* Safe Medication Practice, Regulatory, and Quality Improvement
Knowledge of and active participation in concurrent medication safety event (MSE) monitoring programs, as demonstrated by interventions and RL submissions.

Role Description
We are currently looking for a Pharmacist at 1st Choice pharmacy Management LLC. Objective: Provide comprehensive clinical oncology pharmacy oversight across infusion and oral oncology services by ensuring safe, evidence-based, and patient-centered medication management. Partner with providers and clinical teams to optimize treatment regimens, enhance medication safety, support care coordination, and advance high-quality pharmacy operations across all MOASD locations.
Responsibilities/Essential Functions:
• Collaborate with providers and other clinical staff to provide patients with individualized treatments utilizing the most recent and relevant evidence.
• Review infusion orders for accuracy, appropriateness, and completeness.
• Evaluate and interpret drug related laboratory results. Recommend dose adjustment, addition, or discontinuation of medication(s) as appropriate.
• Identify, resolve, and prevent medication-related problems including subtherapeutic dosage, overdose, drug-drug interactions, drug-disease interactions, adverse reactions, and inaccuracies.
• Conduct comprehensive medication reviews and/or medication reconciliation.
• Perform comprehensive evaluation of patient medical records, to provide proper patient counseling.
• Serve as the clinical contact for questions related to oral oncolytics, side effect management, toxicity monitoring and adherence issues.
• Build, maintain, and standardize treatment regimens within the electronic medical record to ensure accuracy and alignment with current clinical guidelines.
• Review provider requested regimen changes, verify safety and appropriateness.
• Act as the central point of contact for regimen change communication to prevent scheduling errors and ensure infusion schedulers, authorization staff, nursing, and pharmacy staff receive timely information.
• Monitor drug availability issues, guideline updates, pathway changes, NCCN revisions, and proactively update regimens as needed.
• Oversee all aspects of the oral oncolytic program, including verification of new and refill prescriptions, and providing supportive care recommendations.
• Coordinate with pharmacy technicians to ensure timely processing, communication, and follow-up for oral therapy patients.
• Perform drug utilization management activities.
• Ensure compliance with USP 797/800 principles relevant to sterile-compounding oversight in coordination with operational pharmacy staff.
• Oversee pharmacy technician pharmacy related activities.
• Perform final verification of medications prior to distribution to patients.
• Ensure proper storage of medications.
• Research and answer drug inquiries.
• Document appropriately in patient medical records.
• Uphold organization policies, as well as federal, state, and local laws.
• Collaborate with all organizational staff.

Experience, Qualifications, Education
• Graduate of an accredited Pharmacy School with a Pharm.D. Current and unrestricted California State Board of Pharmacy Pharmacist license.
• Currently holds or eligible to obtain California Advanced Practice Pharmacist (APh) licensure within 120 days of hire.
• Must meet ONE of the following:
o Board Certified Oncology Pharmacist (BCOP); or
o Completed PGY2 Oncology Residency and eligible for BCOP (must pass the BPS Oncology Pharmacy Certification examination within 120 days of hire); or
o Completed PGY1 Pharmacy Residency + two (2) years of Clinical Oncology Pharmacy experience (must pass the BPS Oncology Pharmacy Certification examination within 120 days of hire); or
o No residency, but at least four (4) years of Clinical Oncology Pharmacy experience (must pass the BPS Oncology Pharmacy Certification examination within 120 days of hire)

Summary
The Drug Classification & Maintenance Pharmacist participates in the development and maintenance of Core Formulary Processes focusing on the Formulary Maintenance Weekly Drug Update activities including Proxy and CUI maintenance related to the Part D MedImpact Standard and Custom formularies and other functions as required
Essential Duties and Responsibilities include the following. Other duties may be assigned.
• Principal SME Pharmacist on CMS Part D drug eligibility and placement. Maintains and oversees appropriate Part D assignment and CUI mappings of drugs consistent within utilization management groupings of drugs. Reviews new drug categorization changes to customize, implement, and integrate with MedImpact formulary products as appropriate. Monitors drug vendor and regulatory bodies for operational or strategic market changes. Collaborates with the MedImpact National Drug Data File (NDDF) Team as well as MedImpact Drug Information Services to ensure success.
• Performs complex data analytics based upon external drug data provided to MedImpact by FDB, MediSpan, RXNorm and CMS FRF.
• Acts as a clinical and analytical subject matter expert on areas pertaining to Part D drug mapping and classification and supports development and implementation of mapping and PPDL programs.
• Responsible for interpretation and implementation of CMS guidance related to each of the Part D PPDL drug lists. Maintains Formulary Services' CMS Part D Pre-Processing Drug Lists (PPDL) which include weekly review and yearly update CMS_PPDL Drug Lists: The accurate maintenance of lists is essential to CMS Part D compliance and accurate claim adjudication.
• Processes the monthly / yearly CMS FRF and NIH's RxNorm drug update files to ensure accurate and compliant file submission to CMS.
• Serves as Drug Data Management's designated contact for communication to NIH's RxNorm for general inquiries and data correction requests.
• Masters a variety of analytical and quality control tools to ensure formulary drug data accuracy and integrity. Simultaneously engages in the activities and timelines for multiple CMS Plan years with differing and sometimes conflicting rules and regulations.
• Contributes to and assists with current Drug Data Management and formulary enterprise priority projects, programs, or initiatives as defined by leadership. Coordinates inter- and intradepartmental project activities and groups as necessary to meet project goals and timeframes.

Each HCW providing service under this contract or resultant task orders shall undergo an orientation and shall complete mandatory Navy and DoD on-line training as required.
Be able to comply with all applicable Federal, State, and local laws, Department of Defense, Department of Navy, Bureau of Medicine and Surgery, and MTF instructions and policies.
Proficiency in Microsoft Office software applications including Word, Excel, Access, PowerPoint, and Outlook.
MINIMUM QUALIFICATIONS:
Possess and maintain a current certification and registration by the Pharmacy Technician Certification Board (PTCB).
HCW shall be in good standing, and under no restrictions, with the state licensure board in any state in which a license is held or has been held within the last 10
years.
HCW shall be in good standing and under no sanction or suspension by the Federal Government.
Possess and maintain current certification in either the American Heart Association BLS for Healthcare Providers or American Heart Association Healthcare Provider Course.
Able to read, write, speak, and understand the English language fluently and maintain good communication skills with patients and other health care personnel.
Ability to pass all background/suitability checks.
A valid driver's license; ability to travel locally as needed.

Accountabilities: This position involves extensive customer interaction and service. The Product Implementation Analyst installs Pyxis Dispensing systems at our hospital customer sites. These projects typically can range in scope from installing one-unit expansions, to being part of a team that may install projects of over 100 units. The Product Implementation Analyst effectively performs the required tasks to insure successful project completion and customer satisfaction. Home-based position with up to 75-80% daily and overnight regional travel to customer sites in the territory.

Install and support Pyxis Dispensing hardware and software in an acute care setting. Perform upgrades to Pyxis Dispensing software and hardware.
Complete training and develop basic competency in the Pyxis Med Dispensing product line.

Job Summary:

Alert the pharmacist on duty of any medication/product that is not scanning properly
